About this file

This file is a Quality Assurance Surveillance Plan (QASP) for the Defense Logistics Agency (DLA) Disposition Services' Material Handling Equipment (MHE) maintenance contract in the North Carolina Region. The QASP establishes nine key Acceptable Performance Levels (APLs) to measure contractor performance, including visitor log signing, timely preventive maintenance (PM), PM sticker placement, filter dating, PM documentation, corrective maintenance (CM) service, CM cost estimates, timely CM completion, and callback service.

The plan specifies detailed performance standards and timeframes, such as completing PM within 10 business days of notification, providing CM cost estimates within 2 business days of assessment, and completing CM work within 5 business days of approval. Each APL has a maximum allowance of two failures per performance period before payment may be withheld. The Contracting Officer's Representative (COR) is responsible for monitoring compliance and reporting performance issues. The document includes provisions for excusable delays due to circumstances beyond contractor control and establishes procedures for monitoring compliance with Combating Trafficking in Persons requirements.

1. Purpose:

This document is to provide guidance on administering subject contract such that the best possible quality in performance can be assured through appropriate surveillance and addressing of any performance failures as appropriate. The standard APLs are signing the visitor log, timely and complete Preventive Maintenance (PM), PM sticker affixed to each piece of equipment, dated filters, PM sheet, Corrective Maintenance (CM) service, cost estimate for CM service, timely and complete CM and call back service.

3. Surveillance:

a. The primary method of surveillance for evaluating compliance is by Contracting Officer’s Representative (COR) reporting. The COR is responsible for reporting preventive maintenance performance to the Contracting Officer (KO) promptly after the performance deadline has passed. The COR will provide when the action was required to be performed by the contractor and the date it occurred. The COR shall also include what the negative impact of the contractor’s failure to meet the APL was on the DLA DS site.
b. In instances where there are late completions of PM or CM, the COR shall determine if the late performance is caused by an occurrence beyond the reasonable control of the contractor and without its fault or negligence such as, acts of God or the public enemy, acts of the USG either its sovereign or contractual capacity, fires, floods, epidemics, quarantine restrictions, strikes, unusually severe weather, and delays of common carriers. In such cases as there is an excusable delay, the contractor will be deemed to have made the APL for the period.

4. Reduction for Failed APL:

Throughout the performance period of the contract, including options the Contractor will follow the listed APL’s. Any additional failures apart from the listed acceptable performance standards the government may use such measures as partial or full terminations under FAR 52.212-4, not exercising options, and ending a contract early without these measures as long as it is IDIQ and has met its minimum government obligation for the period and a plan for a timely replacement contract is secured.

5. Combating Trafficking in Persons: The COR will monitor the vendor compliance with Combating Trafficking in Persons (CTIP) clause in accordance with DFARS PGI 222.1703(4), such that noncompliance with 52.222-50 is documented to the KO, utilizing and annually completing the CTIP checklist at DoD CTIP Website.
