About this file

This document is a Quality Assurance Surveillance Plan (QASP) Matrix for noxious weed survey and treatment services. The matrix establishes performance standards and quality metrics for contractor performance related to daily treatment records and electronic data collection and submission.

The QASP defines three quality rating levels with corresponding performance expectations and payment consequences. Quality Standard for Rating 1 requires all areas within the defined project area to be surveyed with 90% or better effective treatment on targeted plants, 20% or less loss of non-target species, complete and accurate treatment records submitted within specified timeframes, and electronic data meeting 90% accuracy or greater. Quality Standard for Rating 2 addresses deficiencies found during inspections or unmet contractual obligations, with treatment efficacy between 80-90% requiring rework at the contractor's expense, and payment reductions of 20% if standards are not met after a second inspection or 10% reduction for late or inaccurate treatment records and electronic data submissions. Quality Standard for Rating 3 constitutes unacceptable performance where services fail to comply with contract terms and conditions, resulting in negative performance ratings and no payment if efficacy falls below 80% or if treatment records and electronic data are not provided. Monitoring methods include weekly reviews of Quality Control Plan documents by the Contracting Officer's Representative (COR), periodic onsite inspections by inspectors and/or COR, final inspections at least 14 days after completion to determine treatment efficacy and non-target effects, and office reviews of treatment records and electronic data.
