About this file
This is a Quality Assurance Surveillance Plan (QASP) Matrix that establishes performance standards and quality expectations for noxious weed survey and treatment services. The document defines the requirements for daily treatment record and electronic data collection and submission as a sub-task within the noxious weed survey and treatment task (covering all treatment methods).
The QASP specifies three quality rating levels with corresponding performance thresholds and payment consequences. Rating 1 (Acceptable) requires 90% or better treatment efficacy on targeted plants, 20% or less non-target species loss, complete and accurate treatment records submitted on time, and electronic data meeting 90% accuracy or greater. Rating 2 (Deficient) encompasses performance between 80-90% treatment efficacy, which triggers rework at the contractor's expense; failure to meet standards after a second inspection results in a 20% payment reduction. Rating 3 (Unacceptable) applies when performance falls below 80% efficacy or when treatment records and electronic data are not provided, resulting in zero payment. The matrix also specifies a 10% payment reduction for late submission, inaccurate records, or electronic data completeness below 90%. Monitoring methods include weekly quality control plan reviews by the Contracting Officer's Representative (COR), periodic onsite inspections by inspectors and/or COR, final inspections at least 14 days after completion using ocular estimates and monitoring plots as needed, and office reviews of treatment records and electronic data by the COR.

COR.
Periodic, onsite inspections during performance period by
Inspector(s) and or COR.
Final inspection for treatment efficacy and non-target kill will be determined by ocular estimate at least 14 days after completion.
Performed by Inspector(s) and/or COR. Monitoring plots will be utilized if
Quality Rating 2 or 3 are suspected.

Data by COR.
Deficiencies in performance are found during inspections or other contractual obligations are not met.
Less than 90% but greater than 80% treatment efficacy - rework at contractors expense.
Quality Standards are achieved after rework -positive performance rating and full payment.
Failure to meet Quality
Standards (less than 90% efficacy but more than
80%) after second inspection - 20% reduction in payment (reduction will apply to treatment areas on a case by case basis if task order contains multiple treatment areas).
