PERFORMANCE WORK STATEMENT
Mobilization Force Generation Installation (MFGI) Soldier Readiness Processing Center Fort Bliss, El Paso, Texas
PART I GENERAL
1. GENERAL. This contract provides for personal and non-personal services in coordination with William Beaumont Army Medical Center (WBAMC) with primary support to Fort Bliss Mobilization and Demobilization (MOB/DEMOB), Soldier Readiness Processing (SRP), Conus Replacement Center (CRC), and Separation and Physical History Examinations (SHPE) and secondary support to WBAMC and outlying clinics.
1.1. Mission requirements are to ensure Active Military, Reservist, ARNG, DA Civilians, and Contractor Personnel are medically validated for deployment, upon re-deployment, resolve non-deployable conditions and perform full Separation History and Physical Examinations. All services are to be performed on Fort Bliss military installations, with most services provided within the Soldier Readiness Processing Center and other soldier processing affiliated facilities (medical/dental) located throughout the installation. Current locations are subject to change as the installation’s MFGI Mobilization Support Force (MSF) capabilities and capacities are expected and projected to expand. All services provided under this contract shall be as comprehensive as government-supplied facilities, equipment, and support services permit. The contract personnel may be required to work at different locations within WBAMC due to transitioning services, staff shortages, or peak/surge workloads. The hours required under this contract may fluctuate based on mobilization and demobilization mission requirements. During non-peak times if feasible, the Government will assign contract personnel to support WBAMC’s Primary Care mission. These services shall be essentially the same duties as those required of any DoD Medical Corps, DoD Nurse Corps, Medical Service Corps, DoD Dental Corps, Medical Specialist Corps, or Government Civil Service personnel of similar experience and in similar duty assignments. When not feasible the Government will release contractor’s personnel not required to perform services at the Government's discretion. The Government will notify the contractor when this determination has been made.
1.2. BACKGROUND
As fully detailed in this Performance Work Statement, this requirement is to provide for medical readiness support in coordination with William Beaumont Army Medical Center (WBAMC) to support soldier mobilizations and demobilizations, as well as to provide secondary support to WBAMC and outlying clinics when mission permits.
Department of the Army pamphlet DA PAM 40-502 explains and documents the basic process and general procedures for assessing, documenting, and reporting medical readiness. It contains individual medical readiness (IMR) definitions, assessment, reporting, and monitoring; physical profile definitions, assessment, reporting, and monitoring; medical examinations and assessments; deployment and geographical area considerations. AR 40-502 is the authoritative publication for medical readiness. If any provisions in the DA PAM conflict with the most recent versions of the AR 40-502 policies, the provisions in AR 40-502 take precedence.
Mobilization and Redeployment Concept of the Operations. Support implementation of Combatant Command (CCMD) requirements and accomplish service readiness goals with tasks described in OPORD ORDER 19-20 (Medical and Dental Soldier Readiness Processing) – USAMEDCOM. Providers conducting Soldier Readiness Processing (SRP) and Reverse Soldier Readiness Processing (RSRP) will carefully review all responses on the Deployment Health Assessment (DHA), Periodic Health Assessment (PHA), Separation Health and Physical Examination (SHPE) the electronic health record (EHR), provided documentation, and e-Profile. Providers will address all positive endorsed answers and perceived inconsistencies (obtain medical clearance for moderate or severe responses) with coordination of appropriate follow-up. Medical Provides serving at Mobilization Force Generation Installation (MFGI) comply with standardized procedures outlined in the OPORD Annexes implementing CCMD, Army, and Department of Defense (DoD) specific guidance.
CRC Concept of the Operations. The Conus Replacement Center (CRC) and Individual Deployment Site (IDS) serves as the gateway to check readiness and conduct deployment requirements for the AO. The CRC and IDS validate non-unit related personnel for deployment, resolve non-deployable conditions, provide sustainment of Non- Unit Related Personnel (NRP) flow to the theater, and receive and reintegrate NRP on redeployment. The initial CRC/IDS mission was to ensure Army NRP were fully prepared for deployment. The CRC/IDS mission has expanded in recent years to include individuals from all Services. This includes all Department of Defense (DOD) civilian personnel, contract personnel, and employees of the National Red Cross, and the Army/Air Force Exchange Services (AAFES). The CRC and IDS are activities providing theater specific deployment processing and do not perform a garrison support unit (GSU) or reception station’s responsibility to mobilize Reserve component (RC) soldiers.
SHPE Concept of Operations. The office of the Surgeon General (OTSG), the United States Army Medical Command (USAMEDCOM), and the United States Army Medical community informs members of the Armed Forces on the requirement for a Separation History and Physical Examination (SHPE). The SHPE includes a soldier’s medical history review documented on a DD Form 2807-1 and physical examination documented on a DD Form 2808. Effective 01 Jan 2015, all Active Duty (AD) and Reserve Component (RC) soldiers separating from active duty must complete a separation history and physical exam with the results of that exam entered in their service treatment record (STR) before the date of separation.
Tele-Health Services. As with the past COVID-19 outbreak, or if other public health crises arise in the future, certain personal services as dictated by the Soldier Readiness Processing (SRP) mission may be required to be performed via Tele-Health. Tele-Health is described as the use of electronic information and telecommunications technologies to support long-distance pre-mobilization, mobilization, deployments, and post-deployments. Tele- health services may include, record screenings, interviews, referrals, clinical health care, patient and professional health-related education, public health, and health administration.
Primary Care Services. Is the delivery of integrated, accessible health care services by physicians and their health care teams who are accountable for addressing a large majority of personal health care needs, developing a sustained partnership with patients, and practicing in the context of family and community. Additionally, primary care includes health promotion, disease prevention, health maintenance, counseling, patient education, diagnosis, and treatment of acute and chronic illnesses in a variety of health care settings. Primary care provides patient advocacy in the health care system to accomplish cost-effective and equitable care by coordination of health care services. Primary care promotes effective communication with patients and families to encourage them to be a partner in health care.
MFGI Expansion. The requirements are based upon the Department of Defense’s required flexibility to reconstitute or expand the capabilities of the Military Services on short notice to meet a resurgent or increased threat to U.S. national security.
Health Records. Medical examiners review the Soldier Treatment Record (STR) and/or EHR in use whether an examination is for relief from active duty, resignation, retirement, separation from the Service, or any other physical examination – to include exams done for schools, flight duties and so forth---or as part of a PHA. The examiner notes any significant problems, patient education, and follow-up care on the DD Form 2766 (Adult Preventive and Chronic Care Flowsheet), as appropriate.
Facilities. Contract Personnel must understand and comply with environmental management programs, plans, permits, procedures, policies, directives, and guidance, and comply with all environmental requirements IAW Federal State, and local laws, and Army regulations/guidance.
