About this file

This is a Past Performance Questionnaire (PPQ) issued by the 60th Contracting Squadron at Travis Air Force Base, California for evaluation of contractor performance on a 60 MXS Coverall Laundry Service requirement.

Respondents identified as recent customers of an offeror must complete and submit the questionnaire by 21 April 2026 at 4:30 PM PDT to TSgt Hunter B. Losen, Contract Specialist, at hunter.losen@us.af.mil or 707-424-7736. The form requires respondent information identifying the company/business/military base providing the reference, along with details of the relevant contract performed by the contractor, including contract name/number, description, performance period, annual and total contract values, and whether the contractor served as prime or subcontractor. The evaluation section uses a 1-5 rating scale addressing: quality of service, on-time delivery of service, management of service, communication, contract terms and condition adherance, overall performance, issuance of Corrective Actions Reports (CAR), and willingness to award future contracts. Ratings of 1, 2, or 5 require narrative explanation. The completed questionnaire becomes part of official source selection records and must be submitted as a PDF file. Respondents must provide signature, printed name, and title.
