PAST PERFORMANCE QUESTIONNAIRE
Thank you for taking time to respond to this Questionnaire. The Contractor who forwarded this to you is competing for a contract with the Department of State. The contract will be to provide construction services in Costa Rica. The project consists of the construction of Mobile Border Unit, for delivery in Quito, Ecuador. Be aware that the terms of the contract require the CO to notify the contractor of any adverse ratings (Marginal or Unsatisfactory) in order for them to respond.

Instructions: Section I of the Past Performance Questionnaire to be completed by the Offeror before forwarding to the assessor. The assessor should complete Section II, and submit Sections I and II directly to the Contracting Officer (hwangj2@state.gov.), with a copy to ScottDM@state.gov . Responses are due by March 24, 2023.
