About this file
This is a Past Performance Questionnaire template used by DLA Disposition Services to evaluate contractor performance on previous contracts as part of the evaluation process for new solicitations.
The questionnaire is divided into two parts: Part I is completed by the contractor bidding on the new contract and identifies a previous contract performed, including the contract number, point of contact information, detailed work description, dollar value, subcontractors used, and any quality, delivery, or price problems encountered. Part II is completed by the government agency or commercial customer who managed the previous contract and is returned directly to DLA Disposition Services (matthew.wonch@dla.mil and esteban.m.gancer@dla.mil) by the solicitation closing date. The response section uses a five-point rating scale—Exceptional, Very Good, Satisfactory, Marginal, and Unsatisfactory—to assess five key performance areas: technical performance and quality of product/service; management and key personnel; schedule compliance and timeliness; small business and socioeconomic utilization; and compliance with regulatory requirements. Respondents must provide narrative explanations for each rating and indicate whether they would award to the contractor again. This questionnaire template is used specifically for the DLA Disposition Services solicitation SP4510-26-Q-1014 for Material Handling Equipment maintenance services at Jacksonville and Eglin facilities, which is a small business set-aside IDIQ contract with a 12-month base period plus two 12-month option periods.
