Transmittal Letter to Accompany Past Performance Questionnaire

SUBJECT: Present/Past Performance Questionnaire for [Past Performance Information Submission Selection]

1. We are currently responding to the Defense Advanced Research Projects Agency (DARPA) Technical and Analytical Support Services Request for Proposals (RFP) HR001123R0001 as a [prime contractor or subcontractor to Offeror]. This RFP specifically requires that we, as an Offeror, send a Past Performance Questionnaire to our identified points of contact (POCs) and track its completion and return.

2. We identified the subject contract/task order relevant to this acquisition and you as our POC. As such, please take a few moments to fill out the attached questionnaire and return it directly to the POC indicated below. The information in the completed questionnaire is considered source selection sensitive and cannot be released to us, the Offeror. If you have any questions about the acquisition or the attached questionnaire, your questions must be directed back to the POC below.

3. Please email the completed questionnaire directly to the Government no later than 10:00 am Eastern on Monday, March 13, 2023, at:

Indicates the contractor is deficient and demonstrates very little understanding and ability to perform the goals/objectives of the contract. Such deficiencies are expected to be very difficult or impossible to correct.
Indicates the contractor’s performance meets contractual requirements; however, the area of evaluation experienced serious problems that were difficult to correct. Corrective action was either not identified, appeared only marginally effective, or was not fully implemented.
Indicates the contractor’s performance meets contractual requirements and the goals/objectives of the SOW/PWS.
