About this file
This document is a Past Performance Questionnaire (PPQ) issued by the 60th Contracting Squadron at Travis Air Force Base, California, to evaluate contractor performance for a Dorm Wi-Fi High Speed Internet solicitation.
The PPQ requests that customers of bidding contractors complete and submit the questionnaire by August 27, 2026, at 12:00 PM DST to A1C Elton Edwards (Contract Specialist) at elton.edwards.1@us.af.mil or Ms. Christina Johnson (Contracting Officer) at christina.johnson.32@us.af.mil. The questionnaire consists of three primary sections: Section A requires the offeror's company information including contractor name, address, and point of contact details; Section B requests respondent information identifying the customer providing the performance evaluation; and Section C collects contract details such as contract name/number, description, performance period, annual and total contract values, and whether the contractor served as prime or subcontractor. The performance evaluation in Section C uses a 1-5 rating scale addressing quality of service, on-time delivery, service management, communication, contract terms adherence, and overall contractor performance. Respondents must rate whether the contractor received any Corrective Actions Reports and indicate willingness to award future contracts, with mandatory narrative explanations required for ratings of 1, 2, or 5. The completed questionnaire becomes part of official source selection records.
