About this file
This is a Contractor Responsibility Verification template based on FAR 9.103 and 9.104 requirements. The document serves as a standardized form for prospective contractors to demonstrate their responsibility and qualification to receive federal contract awards.
The template requires contractors to provide evidence or attestation across six key responsibility factors: (a) Financial Resources—documentation such as profit/loss statements, balance sheets, lines of credit, or a statement from a financial institution confirming sufficient resources; (b) Meeting Delivery Requirements—confirmation of ability to comply with required performance schedules considering existing commitments; (c) Performance Record—submission of two minimum past performance examples within the last three years, a capability statement for new entrants, or documentation of previous CPARS and SPRS ratings; (d) Business Integrity and Ethics—certification that SAM.gov responsibility and qualification information is current and accurate; (e) Organizational Structure—confirmation of adequate organizational structure, experience, accounting controls, and technical skills; and (f) Production Capabilities—documentation of technical equipment and facilities or the ability to obtain them. A responsible company official must certify under penalty of perjury that all information provided in factors (a) through (f) is correct and accurate. This verification form does not address specific products, services, pricing terms, response dates, or award dates, as it is a generic template applicable across multiple solicitations.
