About this file

This is a Diversity Practices Questionnaire for the New York Department of Financial Services' RFP C000606 for Audio and Visual Equipment procurement. The questionnaire requires respondent contractors to provide comprehensive information regarding their diversity and supplier diversity initiatives under penalty of perjury. Contractors must submit responses addressing seven key areas: the existence and duties of a Chief Diversity Officer, percentage of gross revenues paid to New York State certified minority- and women-owned business enterprises (MWBEs) in the prior fiscal year, percentage of overhead and non-contract expenses paid to certified MWBEs, provision of technical training to minority- and women-owned businesses, participation in government-approved MWBE mentor-protégé programs, quantitative goals for MWBE utilization in non-government procurements, and formal MWBE supplier diversity program documentation. The questionnaire must be notarized and submitted with supporting documentation where applicable. The RFP establishes a mandatory 30% total MWBE contracting goal, with questions due by October 5, 2026, mandatory intent to bid submission by October 16, 2026, mandatory site visit by October 20, 2026, and final proposals due by November 24, 2026.

All information submitted in response to this questionnaire is subject to audit, and fraudulent statements may result in criminal prosecution and debarment from future contracting opportunities with New York State. Contractors are required to provide specific documentation supporting their claims, including evidence of diversity officer initiatives, program participation agreements, training records, and formal supplier diversity program materials. The responses serve as an evaluation criterion in the contractor selection process, with points awarded based on the comprehensiveness and evidence of the company's diversity practices and commitment to engaging MWBEs in business operations.
