DESCRIPTION OF AGREEMENT

The purpose of this agreement is to provide a means of procuring Occupational Health Surveillance Exams for the North Carolina National Guard. The vendor shall furnish Occupational Health Surveillance services IAW the attached price list, if and when requested by the Contracting Officer.

PERIOD OF PERFORMANCE: _1 Apr 2021_ through _31 Mar 2025_ (4 years)

EXTENT OF OBLIGATION:

The government will be obligated only to the extent of authorized purchases actually made under this blanket purchase agreement by authorized personnel.

PRICING:

Prices will be in accordance with the approved price list that is in effect when the call was made. The vendor shall furnish a price list annually, with beginning and ending dates, to the Contracting Officer.

Price lists will not be effective until signed by the Contracting Officer. The vendor may offer voluntary price reductions for previously approved services without written approval. These reductions should be shown as "VPR" on the invoice.

PLACING ORDERS (RESERVATIONS):

A call is defined as placing a reservation for an event. Only the Contracting Officer or Contract Specialist shall have the authority to make reservations; there are no authorized callers outside the NC USPFO P&C Office.

NOTICE OF INDIVIDUALS AUTHORIZED TO PURCHASE UNDER THIS BLANKET PURCHASE AGREEMENT

AND DOLLAR LIMITATIONS:

A list of individuals authorized to purchase (schedule examinations) under this agreement are identified by name. This list will be revised as needed to provide the most current information, and will be signed by the Contracting Officer.

PURCHASE LIMITATIONS:

Individual calls under this blanket purchase agreement shall not exceed $100,000.00.

Cumulative calls under this blanket purchase agreement shall not exceed $400,000.00.

DELIVERY TICKETS:

All deliveries/services provided under this agreement shall be accompanied by standard original delivery ticket or sales slip. The delivery ticket or sales slip shall contain the following minimum information:

1. Name of Company

2. Blanket Purchase Agreement number

3. Date the call was placed

4. Quantity and unit price less applicable discounts

INVOICE:

An itemized invoice shall be submitted to the individual listed FIRST on the authorized caller letter within 7 calendar days of the event.

TAXES:

All orders placed under this agreement are instruments of the United States of America and the supplies or services ordered thereon are for the exclusive use of an agency of the United States of America.

The Federal Government is not liable for State and Local Taxes.
