About this file
This is a Limited Asbestos Survey Report prepared by ProTeX for Luke Air Force Base, documenting the results of bulk material sampling and analysis conducted on February 12, 2026, at Building 1365 in Glendale, Arizona. The survey was limited in scope to suspect building materials scheduled to be impacted by a planned renovation project.
ProTeX collected twelve bulk samples of suspect building materials, which were submitted to Eurofins EMLab P&K, LLC (Phoenix, Arizona) for asbestos analysis via Polarized Light Microscopy (PLM) in accordance with EPA Methods 600/R-93/116 and 600/M4-82-020. The laboratory is approved by the Arizona Department of Health Services (ADHS No. AZ0964) and accredited by the National Voluntary Laboratory Accreditation Program (NVLAP No. 500031-0). Analysis was completed on February 18, 2026. The survey identified one asbestos-containing material (ACM): samples 2A, 2B, and 2C (black mastic beneath 1" x 1" cream ceramic tiles in the bathroom floor) containing 10% chrysotile asbestos, classified as non-friable Category II material totaling approximately 20 square feet. All other materials tested, including drywall systems (samples 1A, 1B, 1C), vinyl cove base (samples 3A, 3B, 3C), and block and mortar exterior walls (samples 4A, 4B, 4C), showed no detectable asbestos content. ProTeX recommended that any additional suspect materials discovered during renovation or demolition activities be sampled and analyzed prior to disturbance or assumed to be ACM and handled accordingly.

RE: Limited Asbestos Survey ProTeX Job# 17332
Luke Air Force Base
Building 1365
Glendale, Arizona 85309
Dear Mr. Moser:
In accordance with your authorization to proceed, ProTeX has conducted a limited asbestos survey of
Building 1365 located at Luke Air Force Base in Glendale, Arizona. This letter report includes sum-maries of our activities, findings, and recommendations.
PROJECT BACKGROUND
ProTeX was retained by Luke Air Force Base (AFB) Environmental Management to conduct a lim-ited asbestos survey of Building 1365 at the above referenced address. The survey was limited to the suspect building materials scheduled to be impacted by a scheduled renovation project.
ACTIVITIES
On February 12, 2026, Mrs. Sarah Ellwanger, an Environmental Protection Agency (EPA) and As-bestos Hazard Emergency Response Act (AHERA) Certified Building Inspector with ProTeX, collected twelve (12) bulk samples of suspect building materials from the site.
The samples were submitted via chain of custody protocol to Eurofins EMLab P&K, LLC of Phoe-nix, Arizona for asbestos analysis via PLM. Eurofins EMLab P&K is approved by the Arizona
Department of Health Services pursuant to Arizona Revised Statute (A.R.S.) § 36-495.02 (5) to per-form airborne and bulk asbestos fiber analysis (ADHS No. AZ0964). In addition, Eurofins EMLab
P&K is accredited by the National Voluntary Laboratory Accreditation Program (NVLAP No.
500031-0) for bulk asbestos fiber analysis. Sample analysis was conducted via Polarized Light Mi-croscopy (PLM) in general accordance with EPA Method 600/R-93/116 and/or EPA 600/M4-82-020.
The analytical laboratory reports are attached to this letter report.

FINDINGS
According to federal regulations, any material containing greater than one percent (1%) asbestos as determined by PLM is considered asbestos-containing material (ACM). Materials considered asbes-tos containing are highlighted in bold. A summary of the samples collected by ProTeX inspectors is presented in the following table:
CONCLUSIONS AND RECOMMENDATIONS

The test report shall not be reproduced except in full, without written approval of the laboratory. The report must not be used by the client to claim product certification, approval, or endorsement by NVLAP, NIST, or any agency of the federal government. The Company reserves the right to dispose of all samples after a period of thirty (30) days, according to all state and federal guidelines, unless otherwise specified.
All components not quantified as asbestos content and non-asbestos content are considered to be non-fibrous matrix components. Matrix components may include, but are not limited to, gypsum, paint, silicate minerals, vinyl, binder, calcium carbonate, tar, and foam.
Inhomogeneous samples are separated into homogeneous subsamples and analyzed individually. ND means no fibers were detected. When detected, the minimum detection and reporting limit is less than 1% unless point counting is performed. Floor tile samples may contain large amounts of interference material and it is recommended that the sample be analyzed by gravimetric point count analysis to lower the detection limit and to aid in asbestos identification.

Dear Mr.
