About this file

This is a Team Consensus Evaluation Sheet template for evaluating vendor proposals under the HAF Support Services BPA. The document outlines the evaluation process where team members review proposals and document technical evaluations for each vendor using a simple two-tier rating system of "Acceptable" or "Unacceptable."

The evaluation criteria focus on the contractor's Technical/Staffing Plan, including their approach, understanding of requirements, and personnel qualifications. The template provides spaces to evaluate up to three vendors across two main areas: technical/staffing plan understanding and approach (including required resumes), and review of personnel education and experience levels. The document includes definitions for strengths, weaknesses, and deficiencies (noting that any deficiency results in an "Unacceptable" rating), and concludes with a section for the team's recommended vendor and justification. The template is structured to support the evaluation of administrative support services for the Secretary of Defense Business Transformation Office (SAF/AM) as referenced in the associated opportunity FA7014-25-R-0007-v1.
