About this file
This is a Past Performance Questionnaire (PPQ) template issued by the VHA Network Contracting Office 20 in Vancouver, Washington, for use in evaluating contractor past performance on firm-fixed-price construction projects. The PPQ is completed by clients or references of offerors and is not included in the SF330 submission page limit.
Section 1 requires the offeror to provide detailed contractor information including the name, address, point of contact, phone, email, work role performed, and percent of project work performed. It also requires contract information such as contract title, contract number, DO/TO number, contract type, location, award date, original and actual contract acceptance dates, original and final contract prices, and an explanation of any differences between original and final pricing. The offeror must also describe the project scope and explain its relevance to the current solicitation. Section 2 is completed by the evaluator (client reference) and requests evaluator information including name, position, address, phone, email, and role in the project. The evaluator then rates the contractor's performance across seven criteria using a five-point adjectival scale (Outstanding, Above Average, Satisfactory, Unsatisfactory, Not Applicable): Quality, Schedule/Timeliness of Performance, Communication, Management/Personnel/Labor, Cost/Financial Management, Subcontract Management, and an overall Summary assessment including whether the evaluator would hire the firm again. Page 4 provides the rating definitions for evaluator reference and is excluded from SF330 page counts. Completed PPQs must be returned to the offeror for incorporation into the SF330 submission and cannot be submitted directly to NCO 20.
