About this file
This document is a Past Performance Information Sheet (PPIS) template for federal contract submissions. The template requires detailed documentation of prior contract performance, including point of contact information, contract/task order numbers, contractor identification details (CAGE and DUNS codes), service dates, performance locations, and comprehensive service descriptions. Contractors must provide specific labor categories, effort levels (FTEs), detailed service descriptions, and explain similarities to the current solicitation requirements. Additional mandatory sections include the offeror's role in previous contracts (prime/subcontractor/partner), and a description of any problems encountered and corresponding corrective actions taken, which are critical for evaluating past performance credibility and capability.
