About this file
This is a Past Performance Questionnaire (PPQ) issued by the 60th Contracting Squadron at Travis Air Force Base, California for the procurement of 60 MXS Coverall Laundry Service. The document serves as an evaluation tool to gather performance ratings from previous or current customers of bidding contractors.
The PPQ requires respondents to provide detailed information across three main sections: offeror information (the contracting company being evaluated), respondent information (the customer providing the rating), and contract information (details about the specific work performed). Respondents must rate the contractor's performance on a 1-5 scale across six criteria: quality of service, on-time delivery of service, management of service, communication, contract terms and condition adherance, and overall performance. Additional questions address whether the contractor received Corrective Actions Reports (CARs) or similar deficiency reports and whether the respondent would award another contract to the contractor. Ratings of 1, 2, or 5 require narrative explanations. Completed questionnaires must be submitted as PDF files to TSgt Hunter B. Losen, Contract Specialist, at hunter.losen@us.af.mil by 28 April 2026 at 4:30 PM PDT. Receipt confirmation can be obtained by contacting the same email address or calling 707-424-7736. The completed questionnaires will become part of the official source selection records.
