About this file

This is a Past Performance Survey attachment for a Drain & Purge (D&P) services solicitation issued by the Defense Logistics Agency (DLA). The survey is designed to collect information from offerors regarding their previous performance on similar contracts to support the agency's evaluation of contractor capability and reliability.

The survey template functions as an evaluation tool to assess prospective contractors' historical performance on drain and purge operations, equipment servicing, hazardous material handling, and related services. As part of the solicitation SP4510-26-Q-1046, this document would require offerors to provide references, project details, contract values, performance metrics, and customer feedback demonstrating their experience with removing oils, fluids, refrigerants, batteries, and other materials from military equipment including vehicles, generators, trailers, and industrial machinery. The survey supports DLA's evaluation criteria for this 100% Small Business Set-Aside contract, which involves a Firm-Fixed-Price IDIQ time-and-materials contract for drain and purge services across CONUS, Hawaii, and Alaska under NAICS code 562119.
