About this file

This is a Past Performance Questionnaire template used by offerors to document relevant contract experience during federal competitive procurements. A separate questionnaire must be completed for each contract reference, with a maximum length of three pages per contract.

The form requires offerors to provide comprehensive contract details including the customer agency name, address, and point of contact information (name, phone, email). Contract-specific data must include the program title or contract number, contract type (e.g., Firm Fixed Price, Time and Material), period of performance, original contract value, current contract value, estimated value at completion, and clarification of whether the offeror served as prime contractor or subcontractor. Offerors must explain any cost differences between original and current contract values. The narrative sections require: (A) a brief description of effort highlighting relevance to the current acquisition; (B) achievements section addressing quality awards, certifications, customer satisfaction evidence, and metrics; (C) problems encountered and corrective actions, including discussion of quality deficiencies, contract discrepancy reports, cure notices, terminations, disputes, claims, or latent defects; and (D) other relevant information. The offeror bears responsibility for ensuring all point of contact information is current and accurate.
