About this file

This is a Past Performance Questionnaire template that offerors must complete for the Wiring Harness Support Services solicitation (70Z03826RK0000002) issued by the U.S. Department of Homeland Security, U.S. Coast Guard. The questionnaire requires offerors to submit one form per relevant contract, with responses limited to three pages maximum per contract. Offerors must provide detailed information about each past performance reference, including the customer agency name and address, primary point of contact with phone number and email, contract type, period of performance, original and current contract values, and estimated completion value with explanations of any cost variances.

The questionnaire is structured into four sections requiring narrative responses: a brief description of effort highlighting relevance to the current acquisition; achievements demonstrating quality processes, awards, certifications, and customer satisfaction evidence through letters, metrics, or surveys; problems encountered and corrective actions taken, including quality deficiencies, discrepancy reports, cure notices, or disputes; and other relevant information. This template serves as Attachment 4 to the solicitation for a labor-hour contract supporting wiring harness services, anticipated to be awarded on or about May 15, 2026, to a qualified 8(a) Small Business.
