MEMORANDUM FOR QUESTIONNAIRE POCs

SUBJECT: Request for Source Selection Information

1. 48 CONS/PKB is in the process of selecting contractors for solicitation FA558722R0001 for the Fleet Service and Baggage Handling Services for RAF Mildenhall and RAF Lakenheath. Detailed information regarding this solicitation may be found in SAM.gov.

2. One of the considerations in proposal evaluation is the verification of the Offeror’s present and past performance on contracts, which reflects the Offeror’s ability to perform on the proposed effort. We depend on information received from agencies that have had first-hand experience with the Offeror when evaluating the Offeror’s performance.

3. Our areas of interest are summarized in the attached past performance questionnaire. We request your response prior to the time specified in Block 8 of the SF 1449. This schedule will allow our acquisition team sufficient time to analyze the data and complete the source selection process. It is crucial that you return the completed questionnaire directly to the Government.

4. To assist you in preparing your response and expediting your reply, the questionnaire may be filled out electronically and emailed to tawana.Jackson.1@us.af.mil, maria.calimlim@us.af.mil, and bobbie.dobberstein.1@us.af.mil. Your completed questionnaire will become part of our official source selection records.

S Satisfactory Performance meets contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.

M Marginal

Performance does not meet some contractual requirements. The contractual performance of the element or sub-element being evaluated reflects a serious problem for which the contractor has not yet identified corrective actions. The contractor’s proposed actions appear only marginally effective or were not fully implemented.

U Unsatisfactory Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element or sub-element contains a serious problem(s) for which the contractor’s corrective actions appear or were ineffective.

N Neutral Performance was not observed or not applicable to the current effort being reported against.
