About this file
This is a Past Performance Questionnaire (PPQ) template for evaluating contractor performance on the Flight Suits for Blue Angels solicitation (N00244-26-Q-S023). The document is completed in three sections: contractors fill out Section I with contract identification details, previous clients complete Section II with performance ratings, and respondents provide their information in Section III. Contractors must submit one completed form per past performance reference, with each form addressing a recent (within five years) and relevant contract demonstrating similarity in product complexity, manufacturing scope, and contract magnitude. The form emphasizes strict confidentiality and integrity by requiring that completed questionnaires be submitted directly to the Government by the client POC, prohibiting contractors from receiving the completed assessment.
Section II requires respondents to rate contractor performance across four evaluation areas using a five-point scale (Exceptional, Very Good, Satisfactory, Marginal, or Unsatisfactory): Quality of Product (technical specifications conformance, measurement accuracy, workmanship/durability, and material authenticity); Schedule & Delivery (timeliness of deliveries, turnaround times for alterations/repairs, and schedule management); Contract & Business Management (communication, problem resolution, adaptability, and invoicing accuracy); and Overall Performance with narrative assessment of strengths, weaknesses, and recommendation for future contract award. Completed questionnaires must be submitted via email to Navy Contract Specialist Noni Arcilla (noni.p.arcilla.civ@us.navy.mil) and Contracting Officer Naima Yelda (naima.yelda.civ@us.navy.mil) no later than 4:00 PM Pacific Daylight Time on April 30, 2026, with late submissions not evaluated.

N00244-26-Q-S023 Flight Suits for Blue Angels Past Performance Questionnaire (PPQ)
PURPOSE:
This questionnaire is a key part of the Government's evaluation process for the solicitation referenced above. The information you provide will be used to evaluate the past performance of the contractor listed in Section I below. Your candid and timely feedback is greatly appreciated.
Offerors shall complete one (1) Past Performance Questionnaire per contract/order. The Government reserves the right to verify information provided and to contact references.
INSTRUCTIONS
To the Contractor (i.e., the company quoting on the new contract):
1. Completely fill out Section I of this form for a recent and relevant contract you have performed. A separate form is required for each past performance reference submitted.
a. Recent means the contract is currently active or was completed within five (5) years prior to the issuance date of this solicitation.
b. Relevant means the contract demonstrates similarity in the areas of product complexity, manufacturing scope, and contract magnitude.
2. Send this entire form (with Section I completed) to your point of contact (POC) at the client organization for that contract.
3. Instruct your client POC to complete Section II and Section III and submit the form directly to the U.S. Government Contracting Team listed below. To ensure the integrity and confidentiality of the past performance information, the contractor is strictly prohibited from receiving the completed PPQ from their client. Under no circumstances shall the completed questionnaire be returned to the contractor. A violation of this direct submission requirement may compromise the integrity of the evaluation process and may result in the contractor’s quotation being deemed ineligible for award.
To the Respondent (i.e., the Contractor’s previous client):
1. Please complete Section II and Section III of this questionnaire.
2. Your assessment should reflect the performance of the contractor identified in Section I on the referenced contract.
3. Completed questionnaires shall be submitted via email directly to both Government Points of Contact (POCs) identified below. Receipt of the completed questionnaire by the Government POCs is required no later than 4:00 PM Pacific Daylight Time (PDT) on April 30, 2026. Questionnaires received after this date and time are considered late and may not be evaluated. The information you provide will be treated as Source Selection Sensitive information and will not be disclosed outside of the evaluation team.
