About this file

This document is a Monthly Program Report template for the South Carolina Department of Juvenile Justice (SCDJJ) Community Alternatives program, designed to track juvenile rehabilitation and intervention services. The report requires monthly submission by the 5th of each month, capturing detailed statistical and administrative information about juveniles enrolled in community-based alternative programs. The template allows providers to document individual juvenile data, including services received such as Anger Management, Cognitive Behavior Therapy, Substance Abuse Programs, and Vocational Training, along with tracking admission dates, discharge types, and program outcomes.

The report comprehensively tracks key performance metrics including referral and admission numbers, demographic breakdowns, juvenile age and race data, discharge statistics, educational achievements, incident reports, and transition planning details. Providers must document personnel changes, staff training, regulatory inspections, and include a director's signature affirming the report's accuracy. The template also includes a section for denied referrals, requiring explanation for any juveniles not accepted into the program, which enables SCDJJ to monitor program capacity, eligibility criteria, and service delivery effectiveness across community alternative programs.
