About this file

This is a Past Performance Questionnaire (PPQ) template designed for evaluating contractor performance on handyman services contracts at the National Institutes of Health. The document serves as a standardized evaluation tool to be completed by previous clients and submitted directly to NIH procurement contacts Pete Miller (pete.miller@clarion.miller@nih.gov) and Matthew Heavey (matthew.heavey@nih.gov).

The questionnaire is divided into three sections: Section I is completed by the offeror (contractor) and captures basic contract information including offeror name, reference organization details, contract number, project description, contract/task order amount, performance period, and contractor role (prime or subcontractor). Section II requires evaluators to provide numerical ratings (5 = Superior, 4 = Very Good, 3 = Satisfactory, 2 = Marginal, 1 = Unacceptable, N/A = Not Applicable) across 14 performance criteria including ability to manage high-volume small projects, quality consistency, pricing reasonableness, schedule adherence, repair quality, attention to detail, responsiveness to government requests, problem-solving and cooperation, professionalism and communication, disruption minimization in occupied facilities, task prioritization, staffing adequacy, documentation timeliness and accuracy, and safety/security compliance. The questionnaire also includes a yes/no recommendation question for similar handyman services. Section III provides space for evaluators to offer additional comments, particularly explanations for any low ratings. Evaluators must provide their contact information, signature, title, phone number, and email address.
