About this file

This document is an Attachment 4 Cover Form for a Request for Proposal (ITN) issued by the Idaho Division of Purchasing for the design, installation, and purchase of a Video Management System (VMS) to provide surveillance throughout Idaho State Agencies. The VMS solution is to include Network Video Records, Storage servers, Cameras, Network Switches, and related capabilities. The Cover Form must be completed and submitted with the proposal. It requests information on the offeror's legal entity status, tax ID, DUNS number, compliance with the ITN requirements, and affirmations regarding hiring and employment practices. The related state and local contract opportunity is for the procurement of vertical polyethylene liquid storage tanks.

The Cover Form requires the offeror to provide pricing information, indicate any modifications or exceptions taken to the ITN requirements, and confirm the proposal will remain firm for 90 days. It also asks the offeror to affirm it is not suspended or debarred and that it verifies the legal work status of its employees. No information is provided about set-asides, incumbents, funding sources, or other financial details.
