About this file
This is a Conflict of Interest Statement template required for federal contract proposals.
The document establishes compliance requirements under Federal Acquisition Regulation (FAR) subpart 3.6, which prohibits the government from awarding contracts to government employees or organizations substantially owned or controlled by government employees. Contractors must certify their conflict of interest status by selecting one of two checkboxes: either confirming that neither they nor company owners/controllers are government employees, or disclosing that they or a company owner/controller is a government employee. The statement requires signatories to commit to promptly disclosing any actual or potential conflicts of interest that arise during contract performance and to cooperate with any required mitigation measures. Contractors must also acknowledge compliance with all applicable federal ethics, procurement integrity, and conflict of interest regulations, including FAR subparts 3.6 and 9.5, plus any agency-specific requirements. The form must be completed and signed by an individual authorized to legally bind the company, with supporting information including the signatory's name, organization, phone number, email address, and date of signature. The signatory declares under penalty of perjury that all statements are true and correct to the best of their knowledge.
