STATEMENT OF WORK

Introduction

This Master BPA shall ensure Neuromonitoring services are used during surgery to assess the function integrity of the brain, brainstem, spinal cord, or peripheral and cranial nerves. Located at the Alexander T.

Augusta Military Medical Center (ATAMMC), 9300 Dewitt Loop, Fort Belvoir, VA 22060.

Services and Supplies:

1. General. The Contractor shall comply with Federal, State, Local laws, and Federal Regulations as applicable to the performance of this contract.

a. The Contractor shall not accept instructions issued by any person employed by the U.S.

Government, other than: the Contracting Officer (KO), or Bio-Medical Repair (BMR), all acting within the limits of their authority.

2. Scope of Work. The Contractor shall provide experienced, educated and professional neuromonitoring services.

a. “Contractor’s Point-of-Contact” (POC). The Contractor shall provide in writing: the name and telephone number of the primary and alternate English-speaking individual to act as their representative for the scheduling and coordination of service calls and shall be responsible for the coordination of the contract with the Government.

b. The Contracting Officer (CO) shall immediately be notified, in writing, whenever changes are made to the contract.

3. Minimum Qualifications. The minimum qualifications of the technician performing the neuromonitoring services must have the following:

a. Bachelor's degree in Neuroscience, Biology, or Physiology

b. Certification as a Surgical Neurophysiologist (CNIM)

c. 1-Year Prior experience in intraoperative neurophysiological monitoring

d. Proficiency in using neurophysiological monitoring equipment and software, including EEG, EMG, and SSEP systems

4. Identification of Contractor Personnel. The Government (Security-Pass and ID) will provide

‘Identification Badges’ to the Contractor. The ID badge must be worn during the performance of services at the Government site.

5. Government Furnished Property, Materials and Services. N/A

a. The Government will be responsible for maintaining the proper environment, utilities, and site requirements necessary for the service o function properly as specified by the OEM.

b. The Government will operate the service in accordance with the instruction provided in the

OEM manual.

c. During a service visit, the Government will not be responsible for damage or loss due to fire, theft, accident, or other disasters to contractor’s supplies, materials, or personal belongings.

6. Contractor Furnished Property and Material. N/A

7. Replacement Parts. N/A

a. THE Contractor must include software revisions and upgrades (field service changes) required due to a manufacturer announced safety-hazard and FDA recall as part of the contract, at no additional cost to the Government.

8. Contractor Report Requirements. During normal duty hours, Contractor personnel shall check-in with the Security Department, located in Alexander T. Augusta Military Medical Center (ATAMMC), 9300 Dewitt Loop, Oaks Pavilion, Room OL 502, Fort Belvoir, VA 22060, upon arrival at the Government site and again prior to departure. The Security Department can be reached on 571-231- 3814.

a. The Contractor’s FSE shall personally notify (Security Department) of problems that result in the service operator designated by the Security Department.

b. The Contractor shall provide a full-service report upon departure or within two (2) days after completion of all services rendered. Please provide Mr. Daryl Turnage a copy of the surgical report(s) or at daryl.d.turnage.civ@health.mil

c. The service report shall include, but not be limited to contract number, contractors log number, detailed description of the service(s) performed, replacement part(s) information (part number, part-value, nomenclature, unit price, manufacturer). Identify the part as new/used/reconditioned if other than OEM parts are used, as well as the completion date and time, the man-hours expended, the hourly rates normally charged for the type of service performed, and the technicians’ name performing the service.

9. Contractor Responsibility. To ensure precise neuromonitoring is used during surgery to assess the functional integrity of the brain, brainstem, spinal cord, or peripheral and cranial nerves.

a. All charges resulting from a Contractor determine requirement to transport Government owned property covered by this BPA, to and from an alternate repair location shall be the responsibility of the contractor.

10. Equipment Modification Upgrade. The Contractor shall only incorporate OEM specified modification, alteration and upgrades. Approval shall be obtained from the (BMR) prior to the Contractor installation of any modification, alteration or upgrades.

a. The contractor shall perform all OEM field modifications and safety inspections without charge to the Government.

b. The Contractor shall maintain contract with the OEM to determine the requirement for field, modifications to ensure accomplishment of thee field modification in accordance with the time schedule set forth by the OEM.

c. The Government shall not alter the system without prior notification to the Contractor.

d. The Contractor shall perform software upgrade(s) at no additional cost to the Government.

Installation of upgrades will be left to the discretion of the Government.

11. Service beyond the Scope of the BPA. The Contractor shall immediately, but no later than 24 consecutive hours after discovery, notify (Department of Surgery) in writing, of the existence or the development of any defects in/or repair required to the scheduled service that the contractor considers they are not responsible for under the terms of this BPA.

a. At the same time of the notification, the Contractor shall furnish (Department of Surgery) with a written estimate of the cost to make the necessary repairs. Repairs considered by the Contracting Officer to be outside the scope of this PA shall not be covered under this BPA but shall be ordered under a separate purchase order.

12. Safety. The Contractor shall be responsible for knowing and complying with all installation safety, prevention regulations. Suh regulations include, but are not limited to, general safety, fire prevention, and waste disposal.

13. Place/Hours of Performance/Normal Business Hours. The place of performance is located at Alexander T. Augusta Military Medical Center (ATAMMC), 9300 Dewitt Loop, Belvoir, VA 22060.

Contract providers shall perform repair and scheduled maintenance services Monday to Friday, between 8:00am and 5:00pm, excluding Federal holidays.

a. Holidays. The following Federal holidays are observed by ATAMMC:

New Year’s Day Martin Luther King’s Birthday President’s Day Memorial Day Independence Day Juneteeth Labor Day Columbus Day Veteran’s Day Thanksgiving Day Christmas Day

NOTE: Any of the above holidays falling on a Saturday will be observed on the preceding Friday;

holidays falling on a Sunday will be observed on the following Monday. Any holidays that are declare by Presidential Executive Order shall be observed in the same manner as the holidays listed above. If the area in which a contract employee is scheduled to work is closed due too the holiday and the employee is not required to report in, payment will not be made for those hours. Closures of the installations due to inclement weather or tother such acts of God shall be handled in the same manner.

1. The government will measure the Contractor’s performance under the performance objectives listed in the following table by applying the corresponding performance standards and acceptable performance quality level (AQL).

Exceptional: Performance significantly exceeds contract requirements to the Government's benefit.

Satisfactory: Performance meets contractual requirements.

Unsatisfactory: Performance does not meet contractual requirements.

Other performance evaluation factors that will be monitored and documented in the Contractor’s performance records but cannot be quantified by numerical measurements:

Contractor providing personnel exceeding the minimum qualification standards.

Performance above or below the standard, for above factors, will be documented in the Contractor’s Performance Assessment Reporting System 9CPARS) for the reporting period.
