About this file

This is a Past Performance Questionnaire (PPQ) template for the National Institute of Standards and Technology (NIST) Architectural & Engineering (AE) Mechanical/Electrical/Plumbing (MEP) Indefinite Delivery/Indefinite Quantity (IDIQ) Contract. The questionnaire is designed to be completed by assessing officials or references of architectural and engineering firms that have performed work under NIST contracts or task orders.

The form requests comprehensive feedback on contractor performance across nine assessment categories: Quality of Product or Service, Schedule, Cost Control, Innovation/Processes, Management Effectiveness, Key Personnel, Responsiveness/Communication, Overall Performance, and two additional areas at the assessing official's discretion. Responses use a six-point rating scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or N/A) with specific definitions provided for each rating level. The questionnaire also captures contract details including firm name, project name, period of performance, contract award date, awarded value, current contract dollar value, contract type, and contract/order number. Additionally, the form requires identification of the assessing official with contact information and their association with the rated firm. Completed forms must be submitted by email to Sasha Bradford at sasha.bradford@nist.gov. The questionnaire concludes with a question regarding whether the assessing official would recommend the A/E firm for similar future requirements and space for additional comments and signature authentication.
