This document is a template form used by the National Institute of Standards and Technology (NIST) to collect and evaluate contractor past performance information for the AMD TEAM-E/Boulder program. The form is completed in two parts: contractors fill out Blocks 1-4 (contractor information, work performed role, contract details, and project description), while clients complete Blocks 5-8 (client information, their project role, completion date, and signature). Contractors must obtain completed questionnaires from clients and submit them with their proposals to the contracting officer. The questionnaire may be duplicated for future submissions on NIST AMD TEAM-E/Boulder solicitations, and clients are encouraged to submit directly to offerors, though direct submission to NIST is also permitted.

The client evaluation portion uses six adjective rating categories (Excellent, Very Good, Satisfactory, Marginal, Unsatisfactory, Not Applicable) to assess contractor performance across seven major evaluation areas: Quality (technical data preparation, quality standards, problem resolution, quality assurance); Schedule/Timeliness of Performance (delivery compliance, resource utilization); Customer Satisfaction (end-user satisfaction, staff cooperation, customer concern, overall satisfaction); Management/Personnel/Labor (on-site management, workforce qualification, government property control, personnel expertise, small business utilization, multi-project management, requirement changes, overall management effectiveness); Cost/Financial Management (meeting contractual prices, cost reduction innovations, invoice accuracy, accounting system adequacy, contract termination history, financial stability); Safety/Security (safety environment maintenance, security compliance); and General performance factors. The form concludes with a section for narrative remarks addressing strengths, weaknesses, deficiencies, and other comments relevant to evaluating performance risk.
