About this file

This is a Past Performance Questionnaire template issued by the U.S. Government Publishing Office (GPO) Acquisition Services for use in evaluating contractor performance on completed contracts.

The questionnaire is designed to be completed in two sections: the contractor section collects basic contract information including contractor name and address, contract number, contract date, period of performance, place of performance, completion date, contract value, project name, and type of contract, along with a description of the project's key characteristics. The evaluating organization section requires the customer reference to provide the evaluating organization's name and address, the evaluator's name, position during project execution (Project Manager, Contracting Officer's Representative, or other), phone number, and the number of months the evaluator monitored performance. Completed questionnaires must be submitted via email to Contracting Officer Troy White (twhite@gpo.gov) and Contract Specialist Michaela Farmer (mfarmer@gpo.gov). The evaluator must rate the contractor using a standardized response code (EX=Exceptional, VG=Very Good, S=Satisfactory, MG=Marginal, US=Unsatisfactory, N/O=Not Observed) across nine performance categories: quality of products and services, overall performance, schedule adherence, technical requirements fulfillment, cost control, subcontracting compliance, customer satisfaction in routine matters, customer satisfaction in unexpected difficulties, and likelihood of repeat business. The form also includes fields for listing contractor weak points, strong points, other completed projects, and average award fee percentage if applicable. The completed information is classified as source selection sensitive information under 41 U.S.C. 423 and must be protected accordingly.
