About this file

This is a biosecurity questionnaire template required for visitors to the BARC (Beltsville Agricultural Research Center) animal facility. The form must be completed and submitted to Veterinary Services prior to facility entry, with validity lasting seven days from initial submission. Longer-term visitors exceeding 30 days require prior contact with designated veterinarians Dr. Corrin Markey and Dr. Shakera Fudge.

The questionnaire collects visitor information including name, address, job title, visit dates, purpose, specific buildings to visit, and sponsoring employee details. Three substantive questions address biosecurity risks: international travel within 14 days (including farm visits and livestock species), contact with livestock or pet birds within five days, and completion of decontamination procedures (clothing laundering, footwear cleaning, showering, and nasal passage cleansing). Access restrictions apply if visitors had contact with swine or poultry within 72 hours or traveled to certain countries. The form requires visitor and sponsoring employee signatures acknowledging truthful responses and commitment to notify Veterinary Services of any changed circumstances. Completed forms must be emailed to corrin.markey@usda.gov and shakera.fudge@usda.gov.
