INTRODUCTION
2018 ACM & LBP REINSPECTION REPORT
POPLAR BLUFF V.A. MEDICAL CENTER
SCOPE OF WORK – ASBESTOS BUILDING RE-INSPECTION
1. The Inspector and Management Planner reviewed prior inspections and Management
Plan information on file and compared this information with existing conditions for each facility.
2. This asbestos reinspection complies with the requirements of Asbestos Hazard Emergency Response Act (AHERA).
3. Each facility inspection was performed by an accredited inspector for the following buildings at The Poplar Bluff VAMC in Poplar Bluff, MO:
a. Building 1
b. Building 2
c. Building 3
d. Building 4
e. Building 5
f. Building 6
g. Building 7
h. Building 8
i. Building 10
j. Building 11
k. Building 23
4. For each building, the inspector performed the following tasks:
a. Visually re-inspected and reassessed the condition of all friable known or assumed ACBM.
b. Visually inspected material that was previously considered non-friable Asbestos-
Containing Building Material (ACBM) and touched the material to determine whether it has become friable since the last inspection.
c. Identified any homogeneous areas with material that has become friable since the last inspection or reinspection.
d. The inspector collected bulk samples for analysis of newly friable material that is already assumed (not sampled) to be ACBM or collected additional bulk samples to fully assess the suspect materials.
e. Visually inspected, sampled, analyzed, and assessed the conditions of building materials that have been added to the facility since the last inspection or reinspection.
f. Assessed the condition of the newly friable material in areas where samples were collected and of newly friable material in areas assumed to be ACBM.
g. Reassessed the condition of friable known or assumed ACBM previously identified.
h. Recorded above information on field inspection forms for each facility.
i. Inspectors applied appropriate Asbestos Hazard Warning labels/stickers to accessible reaches of known/presumed asbestos TSI piping sections observed during this assessment.
5. A Management Planner reviewed the above information and performed the following required functions:
a. Reviewed each Management Plan for compliance with AHERA and verified that the plan reflects current conditions.
b. Reviewed the results of each reinspection and assessment.
c. Recommended in writing, the appropriate response actions.
d. Signed and dated the review documents, included the accreditation number of inspectors and planners, and submitted a copy to the Designated Person.
6. The following information was submitted to the Designated Person under AHERA, for inclusion in the Management Plan for each facility and, optionally, at a central office location.
a. The date of the inspection, the name and signature of the person making the inspection, state of accreditation, his or her accreditation number, and any changes in the condition of known or assumed ACBM.
b. If samples were collected, the exact locations where samples are collected during the inspection, a description of the manner used to choose sampling locations, the name and signature of each accredited inspector who collected the samples, state of accreditation, and his or her accreditation number.
c. Any assessments or reassessments made of friable material, the name and signature of the accredited inspector making the assessments, state of accreditation, and his or her accreditation number.
7. Asbestos Dust Assessment: This inspection and assessment effort included the investigation and sampling of residual asbestos contaminated dust/debris on ceiling systems in close proximity to damaged asbestos containing TSI piping within Building 1, as observed in the course of asbestos re-inspection efforts. Specifically, surface dust samples were collected from the upper surfaces of accessible (i.e. could be physically accessed for sampling) lay-in drop ceiling tile systems in areas exhibiting damaged or significantly damaged ACM pipe insulation in an effort to assess their impact upon surrounding ceiling tile systems. Findings from this assessment effort have been provided within the summary letter included within this report and dated 12/10/18.
8. Asbestos Building Inspection Limitations - The following limitations apply to this inspection and assessment effort and report:
a. Due to limitations on the ability to penetrate the roofing systems matrix for each building (i.e. risk of voiding existing roofing system warranties paid for by the VA), no roofing matrix samples have been collected to confirm or deny the presence of asbestos containing materials. It should be noted that even recently installed roofing systems can be found to contain ACM, particularly within the flashing materials, as they are currently legally available in the commercial market and should be presumed to have been used unless the architect/engineer responsible for its installation can provide a letter stating otherwise (see Section 3.0). As such, all facility roofing systems are presumed to contain an asbestos containing material until sampled and proven otherwise and should be handled as such by John J. Pershing VA Medical Center maintenance and/or custodial workers as well as outside contractors.
b. Inspection efforts taken during this assessment did NOT include making ceiling and/or wall penetrations into wall/ceiling cavities suspected to have contained ACM, particularly TSI systems, which are commonly found hidden within wall cavities and utility chases. Often, it is not possible to gain visual confirmation of the presence of TSI systems, however, the prevalence of surrounding systems and overall building trends strongly suggest them present. In the absence of visual confirmation, building systems suspected as having ACM TSI on water and/or steam lines have been assumed to be present behind inaccessible barriers until proven otherwise. For example, if a given building exhibits ACM TSI joint fittings on water lines in exposed locations (i.e. hallways), then those same lines feeding into inaccessible wall cavities near bathrooms or shower areas will also be assumed to contain ACM TSI joint fittings. These observations, and associated sampling results, are outlined within the attached report. However, it should also be noted that newer building finishes are often presumed by contractors/maintenance staff as having hidden wall cavity locations that would not be considered as “suspect”, and thus presumed as presenting no asbestos risks, which is a risky presumption. As such, wall cavities that have not otherwise been identified as containing ACM-TSI should be penetrated with caution, and preferably preceded with assessment by a licensed asbestos inspector using destructive means, should future renovation projects call for such demolition activities.
c. Inspection and sampling of flooring systems has been limited to those observations and sampling efforts afforded by “spot” checks performed in a representative number of locations in order to determine the type and extent of suspect building materials and any obvious trends associated with those systems.
For example, if a given room flooring system consists of carpet over one or more layers of floor tile, only a single access point would have been made (typically in the least visible corner of the room) to observe and sample suspect materials. As such, there exists the possibility of unidentified suspect building materials present in concealed, inaccessible locations in other areas of the same room, hall or floor space, and that those materials could be found to be an ACM upon discovery and proper testing. Similarly, if inspectors observed evidence of suspect materials within a given homogenous area, such as second floor tile layers and/or flooring adhesives, then those hidden materials were presumed present throughout the remaining homogenous area based on visible observations of the top flooring layers. As such, there exists the possibility of this inspection identifying an area as having a hidden layer of ACM floor tile and/or mastic which, upon demolition, would reveal a much smaller area of impact.
d. Due to limitations on the ability to collect sufficient suspect adhesive materials typically found beneath ceramic systems (particularly in restrooms and other areas designed for exposure to water), and to do so without significantly damaging the finished ceramic tile surface, samples of ceramic tile mastic/adhesive materials were not collected to confirm or deny the presence of ACM. Adhesives/mastics used for bathroom/kitchen/shower areas commonly contain ACM, are currently available in the commercial market and should be presumed to have been used unless the architect/engineer responsible for its installation can provide a letter stating otherwise (see Section 3.0). As such, all ceramic tile systems should be assumed to be underlain by an ACM adhesive/mastic until inspected and/or sampled and proven otherwise and should be handled as such by VA maintenance and/or custodial workers as well as outside contractors.
e. Inspection and sampling efforts during this assessment did not include the necessary destructive sampling techniques to sufficiently collect samples of fire rated doors as well as heat shields located in the perimeter steam convector units.
Sampling of these materials would have caused a great deal of damage that would have not been able to be repaired. All fire rated doors and heat shields behind the steam convector units throughout the hospital shall be considered to contain asbestos unless sampled proving otherwise.
f. Suspect caulking/glazing materials observed on exterior window systems were safely assessed at ground level only with the assumption that window units at higher elevations are homogenous and carry the same designation with respect to asbestos containing materials.
g. Fixed objects, such as cabinets, lab hoods, etc. often conceal additional layers of older flooring that are not otherwise visible for observation and/or sampling during inspection, but are then uncovered during construction activities.
h. Building systems in constant operation which are critical to the building’s occupation are not capable of being inspected. For example, the interior spaces of an HVAC Air Handler Unit would be considered inaccessible for proper inspection due to safety risks by inspectors.
SCOPE OF WORK – LIMITED LEAD PAINT BUILDING INSPECTION (XRF
SURVEY)
1. Riverfront has performed a limited lead paint inspection of suspect painted/glazed building materials at the Poplar Bluff VAMC in an effort to identify building systems observed throughout the campus suspected as being coated with lead-based paint and/or containing a lead glazing.
2. Specifically, Riverfront used a Heuresis XRF (X-Ray Fluorescence) direct reading instrument (Co-57 Source No. Q5-531; 5 mCi; Model Pb200i; Serial No. 1920) to assess homogenous building systems suspected of containing paints or glazes with elevated lead levels through collection of a representative number of samples of each. This assessment was not intended to provide a surface-by-surface assessment of LBP, but rather a representative assessment of architectural finishes consistently found throughout each building, with a representative number of samples collected from each homogenous component throughout accessible areas in an effort to conclusively identify building systems exhibiting surfaces testing positive for LBP such that future construction projects could be planned accordingly.
3. The following survey strategy was followed for execution of the assessment of target building systems within Building 1:
a. Concrete Walls: 3 shots per floor per unique color combination
b. Concrete Ceilings: 3 shots per floor per unique color combination
c. Plaster Walls - Halls: 15 shots per floor (3/wing) per unique color; plus 1 additional per unique color combination
d. Plaster Walls - Rooms: 15 shots per floor (3/wing) per unique color of paint on walls; plus 1 additional per unique color combination
e. Drywall Walls - Halls: 15 shots per floor (3/wing) per unique color of paint on walls; plus 1 additional per unique color combination
f. Drywall Walls - Rooms: 15 shots per floor (3/wing) per unique color of paint on walls; plus 1 additional per unique color combination
g. Glazed Ceramic Block Walls - Halls: 3 shots per floor per unique color combination
h. Glazed Ceramic Tile Walls - Rooms: 3 shots per floor per unique color combination
i. Glazed Ceramic Tile Floors - Rooms: 3 shots per floor per unique color combination
j. Glazed Ceramic Sinks - Rooms: 3 shots per floor per unique color combination
k. Glazed Ceramic Tubs - Rooms: 3 shots per floor per unique color combination
l. Steel Stairwell Stringers: 3 shots per stairwell per unique color combination
m. Steel Stairwell Rails: 3 shots per stairwell per unique color combination
n. Steel Stairwell Handrails: 3 shots per stairwell per unique color combination
o. Door Frames: 3 shots per floor per unique color combination
p. Window Sills: 15 shots per floor per unique color combination
q. Window Sash: 15 shots per floor per unique color combination
r. Window Frames: 15 shots per floor per unique color combination
4. Lead based paint, as defined by current EPA-HUD protocols, is any surface yielding an XRF result of 1.0 mg/cm2 or greater.
5. The Lead Inspector performed a limited XRF survey of the following buildings:
a. Building 1
b. Building 2
c. Building 3
d. Building 4
e. Building 5
f. Building 6
g. Building 7
h. Building 8
i. Building 10
j. Building 11
k. Building 23
6. XRF survey results for each building can be found within the XRF Lead Survey section of each report package. Lead based paints and/or lead glazed materials testing positive for lead have also been summarized within building and floor specific drawings to illustrate which lead building components are present. This information is provided for the expressed purpose of assisting Facilities Management with project planning and cost estimating with respect to lead building components likely to be disturbed during planned projects.
7. It should be noted that construction histories observed in Building 1 typically revealed evidence of original building systems concealed behind current architectural finishes. In particular, this assessment discovered a consistent trend of original ceramic wall tiles, which consistently exhibited elevated lead levels, being concealed behind new drywall layers, thereby limiting access for full assessment and mapping, and introducing the possibility of hidden lead bearing ceramic wall tiles being overlooked. As such, all existing drywall systems are presumed to contain lead glazed ceramic wall tiles beneath unless destructive inspection efforts prove otherwise.
ASBESTOS MANAGEMENT PLAN RECOMMENDATIONS
The following general notes are for all facilities inspected during the Asbestos Reinspection.
These notes reference any deficiencies found in prior inspections, re-inspections, and the current Management Plan.
1. Removal Priorities were developed for each homogeneous area. The Removal Priorities are an indicator of priorities for future repair work or abatement work to be scheduled as financial budgets allow. Please see the 2018 Reinspection Condition and Removal Priority Table for further detail.
2. The current condition of material was listed for each homogeneous area. The condition, location, and potential for disturbance factors are used for the hazard ranking. Please see the 2018 Reinspection Condition and Removal Priority Table for the condition summaries.
3. Records for custodial, janitorial, and maintenance staff who have received two-hour awareness training should be filed in the Management Plan.
4. Any new custodial, janitorial, or maintenance staff that have the potential to come in contact with asbestos-containing materials (ACM) are required to attend a two-hour awareness training class. Records of training for new employees should be filed in the Management Plan.
5. The Designated Person is required to inform workers and building occupants at least once a year about the Management Plan. This includes information about inspections, response actions, and post-response action activities that are planned or in progress.
Provide a copy of the notification to each facility and include a copy in the Management Plan.
6. Maintenance areas (boiler rooms, janitor closets, etc.) are required to have warning labels where ACM are located. Install warning labels in these areas as needed. Some older labels may have come loose and are no longer visible. Post the labels in a prominent location.
7. Review the enclosed 2018 Reinspection Condition and Removal Priority Table for maintenance actions that may be required and to establish priorities and budgets for future repair and abatement work.
8. Periodic “Surveillance Inspections” are required every six (6) months. This inspection should indicate the condition of the material and provide recommendations for repair or potential removal. Consistency of inspections is desired and, therefore, should be performed by the same individual(s) for all facilities. An accredited inspector is not required, but is recommended. Records of Surveillance Inspections should be filed in both the administrative and facility Management Plans.
9. “Re-inspections” shall be required at the discretion of the local VA facility.
10. The existing Management Plan (original) is updated by this Reinspection. All procedures and methods listed in the Plan are still in effect and should be followed unless specifically updated or omitted by this inspection. This includes maintenance procedures, repairs to damaged material, response actions, record keeping, surveillance inspections, and training.
11. An accredited “Supervisor” should be on staff or available on a contract basis per AHERA guidelines. Certification paperwork should be filed in the training section of the Management Plan.
12. ACBM should only be removed by licensed and accredited contractors in the State of Missouri. Response actions related to abatement activity should be filed in the Management Plan. Removal of limited amounts of asbestos is within the scope of the Management Plan by the accredited Supervisor and trained individuals.
13. Notification should be provided to “outside contractors” about ACM within the facility.
This should include such trades or services as telephone, electrical, plumbing, air conditioning, and other construction work. A form should be created for this purpose and placed in the Management Plan.
14. New building construction and portable buildings added to a facility are required to be inspected and sampled for ACM. In lieu of this, a letter from the architect, engineer, or manufacturer can be obtained that states “no ACM were specified or used in the facility”.
A copy of this letter is to be filed with the Management Plan at the facility. This letter will cancel the requirement for sampling.
15. Abatement Costs were developed for each homogeneous area and per facility. The estimated abatement cost is for budget purposes only. The cost is based on the area/quantity of material and estimated abatement costs for this material. The cost is for abatement only and does not include abatement design, air monitoring, or project observation costs.
