Dobbins ARB Hangar Fall Arrest Systems Statement of Work

April 18, 2022

1. DESCRIPTION

1.1 BACKGROUND: Dobbins ARB is in the process of increasing work safety through improved

Fall Arrest Systems (FAS) so technicians can safely work on all elevated surfaces of C-130 aircraft. Therefore, Dobbins Air Reserve Base is in need of the delivery and installation of four (4) FAS for aircraft hangars (Buildings 838, 731, 746, and 904) to support the maintenance of C-130H aircraft.

The maintenance technicians require access to all areas of the C-130H aircraft, including areas at elevated heights. To be compliant with AFOSH and OSHA standards, proper fall protection systems must be in place in order to protect workers. Installing a new overhead fall arrest systems (FAS) will allow aircraft technicians to easily maintain OHSA approved

100% fall protection coverage, and improve worker safety as well as improve worker efficiency.

1.2 REQUIREMENTS: Dobbins ARB requires a contractor to provide all the necessary parts, equipment, material, labor, and supervision necessary to provide Occupational Safety and

Health Administration (OSHA) and American National Standards Institute (ANSI) compliant fall arrest protections solutions for four (4) aircraft hangars (Buildings 838, 731, 746, and 904) that will support the maintenance of C-130H aircraft.

1.2.1 The contractor shall provide a structural analysis, preliminary, critical and final designs for review and approval by the government prior to proceeding with delivery and installation of each FAS.

1.2.2 Upon approval of the structural analysis and all design submission(s), the government will then provide approval for the delivery and installation for the fall arrest systems for buildings 838, 731, 746, and 904.

Each of the Hangars will require a different Fall Arrest System, as outlined in the

Statement of Work. The contractor shall also provide training on all installed systems.

1.2.3 The contractor shall comply with all commercial and industry standards. All work shall be done in accordance with all federal, local, and state laws and regulations.

NOTE: Track Arrangement Drawings (Attachment 1) are provided for reference only. Actual conditions at the site may differ from those depicted in the drawings. Contractor is responsible for verifying conditions and confirming information provided in drawings prior to beginning work.

2. CONTRACTOR QUALIFICATIONS: The contractor selected for this requirement shall meet or exceed the following minimum criteria:

2.1 Contractor must be a manufacturer of enclosed track rigid rail fall arrest systems.

2.2 Contractor shall have the following individuals included in their design and engineering team for the duration of this project. If either of these individuals identified below are unable to complete the project, the contractor shall replace them with equally qualified individuals and re-submit each new individual’s qualifications for Government acceptance.

2.2.1 FALL PROTECTION PE: A certified PE (Professional Engineer), who is also a

Qualified Person for Fall Protection and specializes in enclosed track rigid rail type fall arrest systems. The Fall Protection PE shall be licensed in the state of Georgia and possess a current PE license and Qualified Person certification. The Fall

Protection PE shall stamp, sign, and certify that the FAS, all components used and the entire safety system as a whole is structurally sound and compliant with all applicable standards and regulations.

2.2.2 STRUCTURAL PE (SE): A certified PE who specializes in building structures. The

PE (SE) shall be licensed in the state of Georgia and possess a current PE license.

The PE (SE) shall prepare the structural analysis of the facility and shall stamp, sign and certify that the existing building structure is able to support the additional loads imposed by the new fall arrest systems (CDRL A0001).

2.3 QUALITY MANAGEMENT CERTIFICATION: The Contractor shall be certified to be compliant with the ISO 9001:2015 quality management system standard. Contractor shall provide proof of current certification issued by a third-party certification authority (registrar).

3. SCOPE

3.1 APPLICABLE STANDARDS: All equipment supplied in the performance of this contract shall meet or exceed the latest applicable standards required for fall arrest systems.

3.1.1 The FAS shall be 100% compliant with applicable Air Force Occupational Health

(AFOSH), Occupation Safety and Health (OSHA), and American National Standards

Institute (ANSI) rules and regulations in effect at time of installation:

3.1.2 All equipment supplied in the performance of this contract shall meet the latest applicable standards as detailed in paragraph 3.1. All applicable new or revised standards that are not in effect at time of Request for Quote (RFQ) closing date but become effective prior to or during system design and installation shall be identified by the Contractor. The Contractor shall notify the Government of any changes that must be made to their system design in order to be compliant with the new/revised standards. In the case of conflicting standards, the most stringent shall apply.

GENERAL REQUIREMENTS

3.2 FALL ARREST DISTANCE and COVERAGE: The system shall be designed and certified to completely and safely arrest a fall from any elevated surface on the aircraft (as defined in each applicable COVERAGE section below) and from any worker position (kneeling, standing, etc.). Special consideration should be given to the case where a worker is kneeling along the leading or trailing edges of the wings and horizontal stabilizers. The Contractor shall verify and document that all fall distance factors (including a clearance margin as described by ANSI

Z359.6) are accounted for.

3.3 SELF RETRACTING LIFELINES (SRL): The SRL’s shall be 3M DBI-SALA Ultra-Lok or approved equivalent. This is a brand name “or equal” requirement.

3.3.1 The Contractor shall provide documentation demonstrating the SRL’s supplied can fully arrest a fall in compliance with the requirements of the “COVERAGE” section.

3.3.2 Each SRL shall have a Type 316 stainless steel cable lifeline connected to a swivel snap hook which attaches to the user’s D-ring.

3.3.3 Each SRL shall have a visual impact indicator on the hook to notify the user if the SRL has been “impacted” and requires servicing prior to use.

3.3.4 Each SRL shall be connected directly to a trolley. There shall not be any cables between the SRL housings and the trolleys.

3.3.5 Each SRL shall have enough cable to extend to the floor of the maintenance stand so workers can connect to the system prior to ascending to the top of the aircraft.

3.3.6 The FAS shall be equipped with the minimum number of SRL’s on each track as required to provide worker coverage. See applicable section under “number of workers in each location” as it applies to each hangar.

Note: The Government desires to maximize the number of workers that can attach to the system and therefore the number of SRL’s available, subject to the limits of the facility to support each system as determined during the course of this contract.

3.4 TAG LINES: Brightly colored ¼ inch diameter, non-metallic tag lines shall be included. Tag lines shall be long enough to reach within 6 feet of the floor of the hangar when the SRL to which it is attached is fully retracted.

3.5 ID PLATE: Each FAS shall have the manufacturers’ name, ID code, serial number, year of installation, contract number and rated capacity on a metal tag permanently attached to the structure.

3.6 SUSPENDED SYSTEM: Each FAS shall be completely suspended from the facility’s roof structure without requiring additional support from the ground. Contractor shall include all brackets and reinforcing as required to properly support the system and the loads it imposes.

3.7 ENGINEERED SYSTEM: The system shall be an “engineered fall protection system” designed with a minimum safety factor of 2 per OSHA regulations. The entire system shall be capable of withstanding shock loads associated with simultaneous fall events in excess of the applicable industry and federal standards in effect at time of installation. The Contractor shall provide stamped drawings as detailed in the “DRAWINGS AND DOCUMENTATION” section below certifying that the FAS and facility structure can support the dead and live loads of the FAS system.

3.8 PE CERTIFICATION: All fall protection drawings created in the fulfillment of this contract shall be stamped and signed by the Contractor’s Fall Protection PE. The Contractor’s Fall

Protection PE shall also provide a certificate for each system which certifies that the system meets all applicable OSHA regulations and ANSI standards. These certificates shall be provided upon completion of installation of each system. (CDRL A001, Certificate of

3.9 PAINTS, COATINGS & SURFACE TREATMENTS: All system components shall have

INDUSTRIAL GRADE corrosion and scuff resistant paints, coatings and/or surface treatments.

Corrosion protection shall be applied at the factory wherever possible so that field applied corrosion protection shall be kept to a minimum. All components shall be protected from damage of the factory applied finishes during shipping, storage and installation. No finished components may be stored exposed to and/or unprotected in the weather. The system shall be painted a safety yellow. All painted surfaces shall be touch-up painted after final installation to provide complete coverage for corrosion resistance and aesthetics. The finishes and coatings applied to the track must not impede the trolley from freely rolling inside the track. Any bumps or irregularities in the structure or finishes or coatings which are large enough to prevent the trolley from freely rolling shall be removed and new paint re-applied.

3.10 NO REPAIR AFTER A FALL EVENT: The FAS, including its tracks and all associated support structures, shall be designed so that there will be no permanent deformation or damage in association with a fall event, nor shall any repair or replacement of components be required after such an event under normal use.

3.11 INTERFERENCE OF FACILITY STRUCTURE, UTILITIES OR FIRE SUPPRESSION

SYSTEM: Contractor shall include any modifications required to the facility to ensure the facility structure and any existing components and/or equipment do not interfere with the proper operation of the FAS.

3.11.1 The FAS shall not interfere with the proper operation of the Fire Suppression

System(s) currently in place. If the contractor identifies any constraints with the design due to the Fire Suppression System(s) placement within each hangar, the contractor shall immediately notify the Government and CO and seek approval before proceeding with the design.

3.11.2 Building utilities are excluded from this requirement however the Contractor shall make every effort to minimize modifications to utilities during the design of the system.

3.11.3 Contractor shall notify the Government of all facility modifications required.

DRAWINGS & DOCUMENTATION

3.12 DRAWINGS & DOCUMENTATION: Contractor shall provide the Government with drawings and documentation as follows:

3.12.1 STRUCTURAL ANALYSIS: The Contractor’s Structural Engineer shall perform a

100% load analysis (field study and design analysis) as required to determine the excess load capacity of the existing structural components within Buildings 731, 746, 838, and 904 to support future FAS loads. (A002, Technical Report-Study (Structural

Analysis)

3.12.1.1 The PE (SE) shall conduct a field investigation of the facility to become familiar with the scope of work to be performed.

3.12.1.2 The Government will not be able to provide detailed drawings of the facility, so field measurements of all structural components of the existing structure are required.

3.12.1.3 The analysis will include all load criteria contained within United Facility

Criteria (UFC) 3-301-01 dated 1 Oct 2019 and American Society of Civil

Engineers (ASCE) 7, latest edition for a new facility.

3.12.1.4 Contractor shall deliver a documentation package stamped by the PE

(SE) which includes field measurements, drawings depicting existing conditions, calculations and narrative of findings and conclusions that certify the facility structure can support all applicable dead and live loads imposed by the new fall arrest systems in combination with other loads as required by ASCE-7 and ANSI

Z359.6.

3.12.1.4.1 The analysis shall consider all loads imposed on the existing structure including, but not limited to, dead loads, live loads, wind loads and seismic loads.

This documentation package will be reviewed and approved by the Government prior to any fabrication commencing.

3.12.2 PRELIMINARY DESIGN DRAWINGS: Contractor shall provide preliminary system design drawings for each FAS. (A003, Technical Report-Study (FAS

Preliminary System Design Drawings)

3.12.2.1 The system design drawings shall show the layout of each FAS above its respective dock.

3.12.2.2 A drawing shall be provided showing the layout superimposed over the outline of a standard length C-130 that includes coverage envelopes for each track.

3.12.2.3 Contractor shall provide a drawing for each FAS that details all points of attachment to the facility structure.

3.12.2.4 Contractor shall provide an overview drawing that shows the preliminary layout of all fall arrest systems in Buildings 731, 746, 838, and 904.

3.12.2.5 These drawings will be presented at the Preliminary Design Review meeting and reviewed and approved by the Government.

3.12.3 CRITICAL DESIGN DRAWINGS: Contractor shall provide critical design review system drawings for each FAS. (A004, Technical Report-Study (FAS Critical System

Design Drawings)

3.12.4 FINAL DESIGN DRAWINGS: Contractor shall provide final system design drawings for each FAS. (A005, Technical Report-Study (FAS Final System Design

Drawings/Final Layout)

3.12.4.1 Contractor shall also provide an overview drawing that shows the final layout of all (8) fall arrest systems in buildings 731, 746, 838, and 904.

3.12.4.2 These drawings will be presented at the Final Design Review meeting and reviewed and approved by the Government.

3.12.5 SHOP DRAWINGS: Contractor shall provide a complete set of shop drawings for each FAS. (A006, Technical Report-Study (FAS Shop Drawings)

3.12.5.1 The Contractor’s Fall Protection PE shall stamp, sign, and certify that each FAS, all components used and the entire safety system as a whole is structurally sound and compliant with all applicable standards and regulations.

3.12.5.2 These drawings will be reviewed and approved by the Government prior to any fabrication commencing.

3.12.6 AS-BUILT DRAWINGS: Contractor shall provide a final set of as-built drawings for each FAS. (A007, Technical Report-Study (FAS As-Built Drawings)

3.12.6.1 The Contractor’s Fall Protection PE shall stamp, sign, and certify that the

FAS, all components used and the entire safety system as a whole is structurally sound and compliant with all applicable standards and regulations.

3.12.7 QUALITY CONTROL: Contractor shall provide a copy of all quality control documents (including mill test reports and other test reports as appropriate) as part of the final drawing package. (A008, Technical Study-Report (Quality Control

3.12.8 OPERATIONS MANUAL: Contractor shall provide the Government with written documentation for the proper operation, inspection and maintenance of the FAS to coincide with delivery of the first unit. (A009, Commercial Off-the-Shelf (COTS)

Equipment Manual Requirements)

3.13 TRAINING: Contractor shall provide the Government’s representatives with training on operation, inspection, and maintenance procedures at the conclusion of the installation of the first FAS. Contractor shall conduct two training sessions in a train-the-trainer format as follows:

3.13.1 Operation and Use Training (for authorized users): (1) class of 10 attendees with class / training time not to exceed 2 hours.

3.13.2 Inspection and Maintenance Training (for qualified maintainers): (1) class of 10 attendees with total class / training time not to exceed 4 hours.

3.14 WARRANTY: Contractor shall provide a written warranty which warrants all products provided under this contract, under normal use, will be free from defects in materials and workmanship for the times specified as follows:

3.14.1 The SRL’s shall be warranted against failure due to defects in materials or workmanship for the term specified by the SRL manufacturer or one year, whichever is greater.

3.14.2 The major structural components, including tracks, mounting brackets, vertical support columns, horizontal support columns, trolleys and carabineers shall be fully warranted against failure due to defects in materials or workmanship for a period of five years.

3.14.3 Warranty shall include all costs of mobilization, shipping, parts and labor.

3.14.4 Warranty period shall start from date of final acceptance of each system. Warranty repairs which present a safety hazard shall be repaired within 7 calendar days of notice of failure. Non-safety related warranty repairs shall be repaired within 30 calendar days of notice of failure.

3.14.5 The warranty period begins when the installation is completed and accepted by the

Government.

BUILDING 731 (AIRCRAFT HANGAR)

3.15 BUILDING 731 (AIRCRAFT HANGAR): The FAS shall be an enclosed track rigid rail style fall arrest system with multiple users connected to each track via trolleys with self-retracting lifeline (SRL) devices.

3.15.1 The FAS shall include parallel tracks which allow workers to pass each other and curved sections of track which allow workers to easily transition from the wings to the fuselage without disconnecting from the system.

3.15.2 The FAS shall include all tracks, trolleys, SRL’s, user harnesses, support structures, associated components and hardware required in order to provide fall protection coverage over all elevated surfaces of the aircraft.

3.15.3 Any and all elements required for complete and fully functioning systems shall be included except for user harnesses.

3.15.4 The Contractor shall initially perform a comprehensive structural analysis of the building and preliminary design review to include preliminary drawings prior to the accomplishing the critical and final design review phase to ensure the facility can adequately support all loads imposed by the fall arrest systems.

3.15.5 The contractor shall provide for the installation and delivery of the fall arrest systems, only after approval has been provided by the Government to proceed.

3.15.6 Contractor shall deliver a final and complete drawing and specification package stamped by a Professional Engineer who is also a Qualified Person for Fall

Protection.

3.15.7 NEW SYSTEM: The FAS and all its components shall be new. Each system shall be ready for operation after installation is complete.

3.15.8 NUMBER OF WORKERS IN EACH LOCATION: The FAS shall provide fall protection for the following number of simultaneous users in each location:

Fuselage (forward of the wings): 4 users

Left wing: 6 users

Right wing: 6 users

Fuselage (aft of the wings): 4 users

Left horizontal stabilizer: 2 users

Right horizontal stabilizer: 2 users

*Note that this section does not constitute a requirement for 24 total simultaneous users on the system

3.15.9 TRACK ARRANGEMENT: A drawing is provided that illustrates the Government’s desired track arrangement for the rigid rail system (See Attachment 1, Track

Arrangement).

Attachment 1, Track Arrangement is a general reference for illustration purposes only and is not intended to be an engineered design that meets all of the requirements of this document.

3.15.9.1 The FAS shall include as many enclosed tracks as required to meet the minimum standards listed in section 3.16.8.

3.15.10 The forward tracks shall include 45 degree transitions with curved corners to allow a worker to turn 90 degrees and transit along the fuselage forward of the wings toward the nose without disconnecting from the system.

3.15.11 The center track shall be continuous from one side to the other to allow a worker to transit the entire length of the wings without disconnecting from the system.

3.15.12 The aft tracks shall include 45 degree transitions with curved corners to allow a worker to turn 90 degrees and transit along the fuselage aft of the wings toward the tail and proceed all the way to the horizontal stabilizers without disconnecting from the system.

3.15.13 RIGID RAIL & TROLLEY SYSTEM: The FAS shall be an enclosed track rigid rail type system with SRL’s connected to trolleys which travel along the tracks. Horizontal lifeline cable type systems are not acceptable and will not be considered for this building (731).

3.15.13.1 Trolleys shall be rated for a 310 lb. user per ANSI Z359.6 with a 3600 lb.

proof load. Trolleys shall be fabricated from corrosion resistant plated steel with

Teflon-impregnated sealed bearings and Teflon-coated wheels for smooth operation over entire length of system.

3.15.13.2 Tracks shall be spaced a minimum of 1 foot apart to allow workers to easily pass.

3.15.13.3 All fabrication welds shall be visually inspected. 20% of fabrication welds shall be inspected using dye penetrant or magnetic particle testing.

3.15.13.4 Joints/connections between segments of the trolley track shall be firmly attached, have minimum gaps possible (1/8 inch or less), and aligned and de-burred for smooth trolley transition from segment to segment. Track system shall be installed level and within manufacturers’ tolerances.

3.15.14 Trolley shall roll freely and follow the user without requiring the user to use their hands to pull or guide the system in any fashion. The SRL shall remain overhead of the worker in order to minimize potential fall distance. The track shall be free of bumps from paint, coatings, rough edges or debris.

3.15.15 System shall be designed such that all SRL’s attached to the system may be in use simultaneously.

3.15.16 System shall be designed such that multiple SRL’s connected to the same track may be in use within 16 inches of each other.

BUILDING 746 (AIRCRAFT HANGAR)

3.16 BUILDING 746 (AIRCRAFT HANGAR): Building 746 requires an overhead Fall Arrest

System (FAS) attached to the existing hangar structure so technicians can safely work on elevated surfaces of C130H Aircraft wings.

3.16.1 The FAS installed in this building shall consist of (4) fixed beam anchors at (2) locations across the top of the wing. Each anchor must be able to support a 310lb worker.

3.16.2 The FAS shall include all anchors, SRL’s, harnesses, support structures, associated components and hardware required in order to provide fall protection coverage on top of airplane wings.

3.16.3 Any and all elements required for complete and fully functioning systems shall be included.

3.16.4 The Contractor shall initially perform a comprehensive structural analysis of the building and preliminary design review to include preliminary drawings prior to the option being exercised for the critical and final design review phase to ensure the facility can adequately support all loads imposed by the fall arrest systems.

3.16.5 Contractor shall deliver a final and complete drawing and specification package

3.16.6 TRACK ARRANGEMENT: A drawing is provided that illustrates the Government’s desired FAS arrangement for this hangar (See Attachment 1, Track Arrangement).

3.16.7 COVERAGE: The FAS shall be designed to provide fall protection for workers on elevated surfaces of the wing surface of the left and right wing of a standard C130 aircraft inboard of #1 and #4 motors.

3.16.8 NUMBER OF WORKERS IN EACH LOCATION: The FAS shall provide fall protection

BUILDING 838 (AIRCRAFT HANGAR)

3.17 BUILDING 838 (AIRCRAFT HANGAR): Building 838 requires an overhead Fall Arrest

System (FAS) attached to the hangar structure so technicians can safely work on all elevated surfaces of standard C130H Aircraft.

3.17.1 The FAS installed in this building shall consist of (8) fixed beam anchors at (4) existing beam locations across the top of the wing. Each anchor must be able to support a 310lb worker.

3.17.2 The FAS shall include all anchors, SRL’s, harnesses, support structures, associated components and hardware required in order to provide fall protection coverage over C130H airplane wings.

3.17.3 Any and all elements required for complete and fully functioning systems shall be

3.18 BUILDING 904 (AGE HANGAR): Building 904 requires an overhead Fall Arrest System

(FAS) so technicians can safely work on all elevated surfaces of Aerospace Ground Equipment

(AGE).

3.18.1 The FAS installed in this building shall consist of (8) fixed beam anchors, (4) will support (2) horizontal cable systems self-retracting lifeline (SRL) devices, the other

(4) will support (4) SRL’s.

3.18.2 Each Cable system must be able to support 2 workers (310lbs each).

3.18.3 The FAS shall include all anchors, cables, SRL’s, support structures, associated components and hardware required in order to provide fall protection coverage over all elevated surfaces of AGE.

3.18.4 Any and all elements required for complete and fully functioning systems shall be

4.2 PRELIMINARY DESIGN REVIEW: The PDR evaluates if design concepts are feasible and technically adequate. The PDR will establish if the system is ready to proceed into more detailed design. The PDR shall be conducted at Dobbins ARB, to be scheduled in advance by the contractor with the Government point of contact (POC). Contractor shall document meeting minutes. 4.3 CRITICAL DESIGN REVIEW: The CDR provides the team with an objective look at what the final design will likely be. The review shall ensure the system can proceed into production and can meet stated performance requirements within cost, schedule, and risk. The CDR shall be conducted at Dobbins ARB or via conference call, to be scheduled in advance by the contractor with the Government POC. Contractor shall document meeting minutes. The CDR presentation material and meeting minutes shall be delivered to the Government within 2 business days of the meeting. (A012, Conference Minutes)

4.4 FINAL DESIGN REVIEW (FDR): The FDR provides one last look at the entire system design before the shop drawings are produced and fabrication begins. The FDR shall be conducted at Dobbins ARB or via conference call, to be scheduled in advance by the contractor with the

Government POC. Contractor shall document meeting minutes. The FDR presentation material and meeting minutes shall be delivered to the Government within 2 business days of the meeting. (A013, Conference Minutes)

4.5 INSTALLATION PHASE: Systems will be installed as the Government’s aircraft maintenance schedule (and therefore, dock availability) allows. Contractor should assume four (4) separate installation mobilization events will be required after approval of the final design review (schedule to be coordinated with Government POC).

4.6 PROJECT STATUS MEETINGS: Project Status Meetings shall be held every two weeks during the installation phase of the project. These meetings may be held in person or per teleconference. Contractor shall be responsible for providing the conference calling service.

6. PRODUCT CONFORMANCE

6.1 Product conformance: Systems shall meet salient characteristics to conform to producers own drawings, specifications, standards, and quality assurance practices, and be the same product offered for sale in the commercial marketplace. The government reserves the right to require proof of such conformance before first delivery through submission of contractor’s commercial manuals, commercial literature. (A016, Commercial Off-the Shelf Equipment

Manual Requirements)

6.1.1 Products manufactured to metric dimensions will be considered on an equal basis with those manufactured using inch-pound units, provided they fall within the specified tolerances using conversion tables contained in the latest revision of FED-

STD-376, and all other requirements. If a product manufactured to metric dimensions exceeds the tolerances specified in the inch/pound units, a request should be made to the contracting officer to determine if the product is acceptable.

7. FINAL INSPECTION AND ACCEPTANCE:

7.1 The acceptance test shall be conducted by the contractor’s personnel and witnessed by a member of the 94 MXG or designated representative. The Contractor shall perform on-site, 310 pound drop endurance tests on two sacrificial SRL devices. The systems will be examined for design, construction, materials, components, and workmanship to determine compliance with the contract and this specification. Any failure in a drop test or deviation from the manufacturer specifications shall be cause for rejection. If the results of the test are unacceptable, the contractor shall have fifteen (15) business days to satisfactorily demonstrate an acceptable, operational system.

8. SHIPPING

8.1 Shipping costs. The Contractor shall pay all shipping costs, FOB Destination.

9. GOVERNMENT FURNISHED PROPERTY OR SERVICES

9.1 The government will provide electricity, water, restroom facilities and limited storage. No other items will be furnished.

9.2 Other Government Services. The Government will ensure sufficient floor space is immediately available to the Contractor when they arrive to perform the installation and training.

9.2.1 The Government will provide temporary storage space sufficient for Contractor to store parts, materials and equipment required during each installation event.

9.2.2 Contractor shall not store any parts, materials or equipment on Government property between installation events. No other Government property or services will be provided.

9.3 Waste. The Government will not provide garbage and/or material disposal services.

Government dumpsters may not be used for disposal of Contractor’s waste materials. The waste becomes the property of the Contractor and may be disposed in accordance with all applicable state, federal and local statutes, regulations, and executive orders in effect at the time of disposal. Contractor shall be responsible for removal and disposal of all waste material during each installation event.

9.4 Work Area: The work area shall be maintained in an uncluttered and safe condition at all times.

9.5 Equipment. The contractor shall provide all equipment necessary to perform the requirements of this contract. Under no circumstances shall the contractor use any

Government equipment (including, but not limited to lifts, ladders, stools, tools, etc.) during installation of deliverables under this contract.

10. GENERAL TERMS

10.1 Government Representative. The Contractor shall coordinate all work activities with the following Government Representative:

10.2 Contractor’s Representative. The Contractor shall provide the Government

Representative with the Contractor Representative’s contact information to include telephone number and email address. This information shall be provided to the Government

Representative via email within two (2) days of award.

10.3 FOD Requirements. Contractor shall adhere to AFI 21-101(section 11.8.), Foreign Object

Damage (FOD) Prevention Program. The purpose is to eliminate the introduction of any foreign object that could cause damage to any aircraft. FOD can be caused by not accounting for hardware, safety wire, tools, supplies, etc., during operations and maintenance. Any or all

- loose objects, regardless of their origin, can cause catastrophic and costly damage to an aircraft or major end item. These regulations apply to contracted services where work is performed in, on, around, or requires travel through areas near aircraft, support equipment, engines, or components including personnel operating vehicles and equipment on AFRC flight lines, runways, taxiways, amps, and in aircraft hangars or maintenance areas.

Prevention of FOD is a critical item, which must be accomplished by the contractor during the on-site portion of this task.

10.4 Hazardous Chemicals. The contractor shall not release any hazardous or dangerous materials during the execution of this contract. All chemicals, cleaning agents, or other liquids used must have Material Safety Data Sheets readily available in case of accidental spill or contact with eye/skin. Contractor shall immediately notify the Government POC of any hazardous or dangerous situation as work develops.

10.5 Environmental & Hazardous Materials Handling: The contractor must present a material Safety Data Sheet (MSDS) for any chemicals brought on base for the purpose of fulfilling the requirements of this contract. This includes materials such as oil, grease, and paint.

10.6 Fire extinguishers. Adequate number and type of portable fire extinguishers, supplied by the contractor, will be as close to the work as possible. At least two 10 lb ABC type fire extinguishers shall be located at close proximity to the work being performed at all times.

The fire inspector issuing the welding/burning permit will determine the required level of protection at the time of permit issuance.

10.7 Outages. Interruption of service for HVAC, Power, Lighting, telephone, Fire Protection, Fire Detection, Water, Sewer, Roads, and Parking Lots are considered outages. Outages shall be scheduled at least one week prior to the desired time. (Outages to be coordinated with

Government POC).

10.8 Contractor Equipment: The contractor is responsible for the maintenance, fueling, and safety of their equipment, tools, vehicles, and personnel.

10.8.1 When refueling, lift trucks shall be refueled outside of the hangar.

11. SECURITY & SAFETY:

11.1 Security: The contractor shall coordinate with the 94 MXG or designated representative to obtain temporary badges. Contractor personnel shall comply with established security procedures for entering an installation and its facilities, including special security procedures for entry to restricted or controlled areas.

11.2 Safety: The Contractor shall comply with all local safety regulations and procedures in effect on-site.

11.3 The Contractor is solely responsible for compliance with Occupational Safety and Health

Administration (OSHA) standards for the protection of their employees. The Government is not responsible for ensuring that Contractors comply with “personal” safety requirements that do not present the potential to damage Government resources.

11.4 The Contractor is responsible for the safety and health of their personnel and protection of the public on Government work sites (DODI 6055.1, Paragraph E5.1). The Contractor shall maintain a health and safety program that meets Occupational Safety and Health

Administration (OSHA) standards. If the Government notifies the Contractor of a potential

OSHA violation, the Contractor is obligated to comply with the applicable OSHA regulations.

11.5 Security Incident or Violation. The Contractor shall immediately notify the Government

POC of any potential or actual security incidents or violations.

11.6 Safety Mishap/Incident Reporting. The Contractor shall report all mishaps/incidents to the government POC. The Contractor shall immediately secure the mishap scene and damaged property and impound pertinent maintenance and training records until released by the Government’s Safety Office (94AW/SE). Such release shall be accomplished through the CO. The Contractor shall cooperate and assist Government personnel in the investigation of the incident and submit an Accident/Incident Report within 24 hours of the accident/incident. The Contractor shall require all subcontractors to comply with required safety, health and fire standards.

11.7 Operations Security (OPSEC). The Contractor shall ensure Contractor personnel who perform work at Dobbins ARB or another Government facility comply with the OPSEC procedures of the facility.

12. Place of Performance: Dobbins Air Reserve Base

13. Work Schedule. The on-site services shall be performed between the hours of 06:30 am and

3:15 pm (EST), Monday through Friday, excluding Federal holidays. Working any hours outside this period shall be coordinated with the Government Representative and approved by the

Contracting Officer.

14. Environmental Management System (EMS). Environmental Management Systems (EMS)

Awareness Training: All Contractor personnel working on Dobbins ARB who perform activities on the installation are required to complete Air Force-provided initial EMS Awareness Training.

It is the responsibility of the Prime Contractor to ensure that all sub-Contractors, vendors, and employees complete this training prior to beginning work on Dobbins ARB. Notification of training completion shall be sent to both the Dobbins ARB Contracting Officer (CO) to be maintained in the contract file and Environmental Management, 94 MSG/CEV, (susan.eason@us.af.mil) for tracking purposes. Failure to provide documentation of EMS

Training may result in termination of the contract.

15. Contractor’s Safety Requirement. The Air Force is part of the OSHA Voluntary Protection

Program (VPP). All shall familiarize themselves with the requirements of VPP. Information on the

VPP can be accessed at http://www.osha.gov/dcsp/vpp/index.html. Additionally, the Contractor is responsible for the safety and health of all subcontractor employees.

15.1.1 Safety and Health Plan. Within 10 calendar days after contract award, the

Contractor shall provide a copy of their company Safety Plan that is required to ensure compliance with the Occupational Safety and Health Act (OSHA) (Public Law

91-596). The Safety Plan will be provided to the Safety Office for review. The Safety

Office will provide acceptance or non-acceptance within 5 working days of receipt to the Government point-of-contact who will notify the CO.

15.1.2 The Contractor will have 5 working days to resubmit the Safety Plan, amendment or addendum to address or correct issues identified for non-acceptance. The Safety

Office will provide acceptance or non-acceptance of the resubmission within five workings days of receipt to the Government POC who will notify the CO.
