About this file

The document is a Project Experience Questionnaire template designed to collect detailed performance evaluation information for contractors involved in aerial treatment (spraying) programs. The two-page form is divided into two primary sections: Part A for Contractor Information and Part B for Client Information, with a comprehensive rating scale for assessing contractor performance across 11 different evaluation criteria.

Part A requires contractors to provide company details, contract specifics, and a project description explaining how their previous work relates to the PPQ aerial treatment program. Part B, to be completed by the client, includes a detailed rating scale ranging from Exceptional (E) to Unsatisfactory (U), allowing clients to evaluate the contractor's performance on aspects such as contractual compliance, quality standards, workforce and equipment resources, management effectiveness, safety, and overall cooperation. The questionnaire is specifically tailored for the USDA APHIS Plant Protection and Quarantine Services' Grasshopper and Mormon Cricket (GHMC) Suppression Program, which seeks to establish multiple Indefinite Delivery Indefinite Quantity (IDIQ) contracts for insecticide spraying across states including Nevada, Oregon, Wyoming, Colorado, and Utah.
