About this file

This is a questionnaire form (Q-48OP) for vendors responding to an Invitation for Bid (IFB) issued by Globe Unified School District (GUSD) in Arizona to provide delivered and offsite fuel, oil, and similar products. The questionnaire is structured in five question sets containing a total of 81 questions that vendors must complete and submit electronically through the Bonfire portal. The form requires vendors to provide comprehensive information regarding company profile, experience, method of approach, pricing, and an affirmation statement. The questionnaire must be submitted in the specified Excel format without modifications to its structure, and vendors are instructed to use text-based responses only, avoiding special characters, formulas, or alternative file formats.

The questionnaire addresses critical operational and compliance requirements, including vendor financial stability, fleet capacity, delivery procedures, fuel quality standards, and fuel card/fleet card capabilities. Vendors must warrant that all fuels meet federal, state, and local standards, with specific requirements for unleaded 87-octane gasoline compliant with ASTM D4814 standards and diesel fuel conforming to ASTM D-975 specifications with ultra-low sulfur content. Pricing must be based on the Oil Price Information Service (OPIS) benchmark using Arizona rack locations, with a firm discount percentage or differential for the contract term. Deliveries are to occur Monday through Friday between 7:30 AM and 3:30 PM Arizona Time to GUSD's location at 455 North Willow Street, with vendors required to provide weekly OPIS reports and comprehensive documentation including bills of lading and delivery tickets. Vendors must also demonstrate compliance with environmental regulations, possess appropriate licensing and certifications for fuel transport, conduct background clearances and drug screening for staff, and maintain Certificate of Insurance with GUSD listed as an additional insured.
