Summary of Attachment 2 - Pricing Submittal Form

This is a pricing submittal form for a Request for Quote (RFQ) issued by the Oregon Department of Forestry (ODF) for the Crescent Lake Bark Beetle Non-Commercial Thinning project (RFQ S-62900-00016365). The project involves furnishing all equipment, supervision, and labor services for non-commercial thinning and removal of damaged and diseased trees on private land within the Klamath-Lake District in Crescent Lake, Oregon. The contract encompasses an estimated 49 acres of work; however, this quantity is not binding and the ODF reserves the right to remove acres during the Intent to Award phase to remain within budget or to award fewer quantities than estimated. Contractors must be licensed by the Bureau of Labor and Industries (BOLI) as a Farm/Forest Labor Contractor throughout the contract duration. The document specifies that offerors must provide unit pricing for all items, with award determined by the lowest total amount submitted by a responsible and responsive bidder.

Pricing must include all travel, per diem, transportation, and other expenses with no separate compensation provided for travel time, move-in, move-out, or demobilization costs. Payment will be made only for inspected acres that meet acceptable work quality standards. The ODF reserves the right to award all, part, or none of the offered items based on considerations including price, experience, expertise, product functionality, suitability for purpose, and contractor responsibility under Oregon Revised Statutes 279B.110. Bidders assume all risk of site investigation and are solely responsible for their cost estimates; the ODF does not warrant the accuracy of any provided estimates or information regarding land characteristics, conditions, or density of the treatment area.
