NAME OF CONTRACTOR REQUESTING THE REFERENCE:
The purpose of this questionnaire is to obtain information regarding the quality of the above named Contractor’s past performance relative to a contract, completed or in progress, at your company/agency. NASA Armstrong Flight Research Center’s Request for Proposal No.
80AFRC20R0003 requires Offerors to identify current or previous customers and solicit their response regarding our performance.
It is requested that the individual(s) responsible for the administrative oversight of the project
(e.g. COR, project manager, etc.) respond to this questionnaire.
On or before May 22. 2020 4:00 pm Pacific Time, please email the completed survey to the NASA Points of Contact listed below.

1. How consistent is/was the Contractor in understanding the customer’s requirements and providing sufficient staffing?
2. Was the Contractor able to build effective working relationships with associate contractors, subcontractors and the government in a team environment?
3. How effective is/was the Contractor in identifying, responding and resolving problems?
4. How is/was the Contractor’s emphasis on safety protocol and responsiveness to mishap investigations (if applicable)?
5. Discuss the Contractor’s ability to provide sufficient management to ensure effective performance over the course of the contract.

____ Good/Satisfactory: Effective performance; fully responsive to contract requirements;
reportable deficiencies, but with little identifiable effect on overall performance.
____ Fair/Marginal: Meets or only slightly exceeds minimum acceptable standards; adequate results; reportable deficiencies with identifiable, but not substantial effects, on overall performance.
____ Poor/Unsatisfactory: Does not meet minimum acceptable standards in one or more areas;
remedial action required in one or more areas, deficiencies in one or more areas which adversely affect overall performance.
