About this file
This is a Past Performance Questionnaire (PPQ) template used by the Defense Health Agency (DHA) Western Markets Contracting Division to evaluate contractor performance on federal contracts. The form is designed to be completed by project references who worked directly with the contractor being evaluated.
The questionnaire collects evaluator information including name, organization, contact details, and position held in relation to the project. It requests offeror information and detailed project information such as contract number, delivery/task order number, contract title, location, award date, completion date, contract value, and whether the offeror performed as prime contractor or subcontractor. The form contains 19 evaluation criteria using a standardized five-point rating scale: "E" (Exceptional), "VG" (Very Good), "S" (Satisfactory), "M" (Marginal), and "U" (Unacceptable), with "NA" for not applicable. Evaluation topics include relationship with client, subcontractor management, employee retention, corporate integrity, work quality, quality control, schedule compliance, cost control, labor and safety standards compliance, and documentation submission. The form also includes yes/no questions regarding cure notices, performance problem resolution, likelihood of future contract award, customer satisfaction, and opportunities for the contractor to respond to negative comments. Completed questionnaires are to be submitted to angelinemae.m.santiago.civ@health.mil no later than July 2, 2026. The government reserves the right to verify all information and may advise contractors of adverse remarks while protecting the confidentiality of reference provider identities per Federal Acquisition Regulation requirements.
