About this file

This is a Past Performance Information Form template designed to collect relevant contract history from offerors responding to the solicitation for Academic Instructor and Tutor services at Naval Recruit Training Command (RTC) Great Lakes, Illinois. The form requires offerors to document their three most relevant past performance contracts, including customer name and location, points of contact with verification information, total dollar values broken into annual increments, period of performance (with a minimum of one year completed prior to solicitation issuance), and average full-time equivalent personnel assigned per year. Offerors must provide detailed descriptions of work performed that demonstrate relevance to the current solicitation scope, with specific metrics required for helpdesk work such as user population supported, number of trouble tickets processed, and average turnaround time.

The form includes two critical notes for subcontractor involvement. If a proposed subcontractor's past performance is submitted, the offeror must clearly state the percentage and type of work the subcontractor will perform on the current requirement and detail the specific aspects of the solicitation work assigned to that subcontractor. Additionally, if the prime offeror or subcontractor served in a subcontractor capacity on a referenced past performance example, they must clearly document the work they personally performed and provide the dollar magnitude of only their subcontracted work, excluding the prime contractor's total contract value.
