HVIP Budget Template Summary

This is a budget template document supporting a grant application for Hospital Based Violence Intervention Programs (HVIP) under HB4045, administered by the State of Oregon. The template is designed to assist applicants in preparing budgets for seed grants that will support planning and development of HVIPs outside Multnomah County and existing geographic catchment areas. The funding seeks to establish partnerships between Community Based Organizations (CBOs) and health systems providing emergency room and urgent care services, with the ultimate goal of creating a statewide learning community committed to exploring HVIP partnerships and planning future implementation.

The budget template provides a standardized format for applicants to detail project costs and funding requirements. This tool enables prospective grantees to present comprehensive financial proposals aligned with the funding opportunity's objectives of supporting partnership development and program planning activities. The template facilitates consistency across applications and allows the administering agency to evaluate proposed budgets against available funding resources and project scope requirements.
