About this file

This document is a Past Performance Questionnaire (PPQ) to be used in evaluating an offeror's past performance for a federal contract opportunity. The PPQ requests information from the offeror's customer, including the customer's point of contact, contract details such as contract number, type, and period of performance, as well as a description of the services provided. The customer is asked to assess the offeror's performance in several areas using a rating scale from Exceptional to Unsatisfactory. The related federal contract opportunity is for providing service/maintenance, calibrations, and regular testing for the Naval Hospital Bremerton. This is a Request for Quote (RFQ) for a non-personal services contract with a performance period of base year plus four option years. The contract will be awarded using simplified acquisition procedures and is an unrestricted procurement.
