About this file

This is a Darfur Certification form required by the California Department of Social Services and Department of General Services for IFB 24-3089. The document outlines certification requirements under Public Contract Code Sections 10475-10481 for companies that have had business activities outside the United States within the previous three years.

The form provides two options for vendors: Option #1 allows companies to certify they are not a scrutinized company as defined in Public Contract Code 10476, while Option #2 requires written permission from the Department of General Services if the company is deemed a scrutinized company but wishes to bid on the contract. Companies that have not conducted business outside the US in the past three years are exempt from completing this form.
