About this file

This is a Privacy Act Statement form issued by the Air Force Research Laboratory (AFRL) for use in federal research and development solicitations.

The form establishes the legal authority and purpose for collecting personal information from individuals and institutions submitting proposals for federal grants, cooperative agreements, contracts, and Other Transaction Agreements administered by the Department of the Air Force. The statement outlines that collected information may be used for processing, investigating, and maintaining records to ensure awards are made to responsible parties and entities. Information sharing is authorized to contractors, grantees, experts, consultants, and others performing work on federal contracts or grants, as well as to appropriate federal, state, local, and international law enforcement authorities when records indicate potential violations of law. The form specifies that while providing information is voluntary, 42 U.S. Code § 6605 imposes mandatory disclosure requirements for federal research and development awards, with enforcement mechanisms including civil or criminal remedies and rejection of non-compliant applications. The document includes signature blocks for a covered individual and the institution's authorized representative to acknowledge consent, along with fields for proposal title and institution name. This form serves as a template document required for AFRL research solicitations.
