About this file

This is an attachment to a Request for Proposal (RFP) solicitation document issued by the Arizona Department of Administration State Procurement Office for Statewide Mobile Diagnostic Imaging Services. The solicitation seeks to establish one or more statewide contracts to provide mobile diagnostic imaging services, including X-Ray, Ultrasound, and Echocardiogram technologies, to all Arizona state agencies, universities, boards, commissions, and participating purchase cooperative members. The RFP was posted on August 25, 2025, with a proposal submission deadline of October 3, 2025, allowing approximately six weeks for vendor response. The specific attachment provided is a "Method of Approach" evaluation form that requires offerors to provide detailed written responses addressing six primary categories: method of approach for service delivery, equipment specifications and maintenance, staffing and training protocols, HIPAA and cybersecurity compliance, customer satisfaction monitoring processes, and identification of proposed subcontractors.

Offerors must provide comprehensive responses to all evaluation questions, with failure to answer fully potentially resulting in a non-responsive determination. The method of approach section requires vendors to describe patient capacity per site visit, geographic service areas within Arizona, and critical findings notification procedures. Equipment responses must detail mobile imaging unit specifications, calibration and maintenance schedules, and backup unit availability. Staffing inquiries address technologist and driver staffing models, ongoing training for safety and compliance, credentialing and privileging processes, and staff training for vulnerable populations. Security responses must address HIPAA compliance measures and cybersecurity protocols for data transmission and storage. Customer satisfaction responses require documentation of ongoing communication and oversight mechanisms as well as escalation procedures for problem resolution. The file emphasizes that offerors bear sole responsibility for submitting required information and that copying the Scope of Work in lieu of unique responses is strongly discouraged.
