About this file

This is an Indian Owned Economic Enterprise Representation form issued by the United States Department of the Interior in July 2013 for use in federal solicitations.

The form serves as a self-certification provision that offerers must complete to represent whether they meet the definition of "Indian Owned Economic Enterprise" as defined in 1480.201, in accordance with 25 USC 47 (the Buy-Indian Act). Offerers must certify their status by checking either "does" or "does not" meet the definition. The enterprise must maintain eligibility throughout three critical time periods: at the time an offer is submitted in response to a solicitation, at the time of contract award, and during the full contract performance period. Any loss of eligibility at any point requires immediate written notification to the Contracting Officer. The form requires completion of four fields: Solicitation/Contract Number, Project Number/Title, Date, and offerer information including Business Name, SAM UEI Number, Certifying Signature, and Print Name. The document includes penalties for false or misleading information, including violations under 18 U.S.C. 1001 for false representations in offers and liability under 31 U.S.C. 3729-3731 and 18 U.S.C. 287 for false claims during contract performance.
