1. Introduction

1.1. SCOPE. Unless otherwise specified in this Statement of Work (SOW) the Contractor shall provide all supervision, personnel, equipment, tools, transportation, materials, supplies, off base storage and disposal as required to perform repair/replacement of the raised access flooring back to original conditions in Building 153 computer server rooms. The contractor shall provide material to include but not limited to floor panel, stringers, pedestals, pedestal adhesive, edge strip, pedestal caps, and other supplementary materials as needed and approved by the Government. The contractor will not be required to suspend or lift any equipment resting on the raised floors. All products shall be stored and installed per manufactures guidelines. All work shall be in accordance with applicable federal, state, and local regulations, specifications, commercial practices, industry standards, and this SOW.

At completion of the project, the Contractor shall remove all materials waste unless specified in writing by the Government. This introduction describes materials, work in general terms, and shall be used in conjunction with the entire SOW. Work includes but is not limited to the following:

1.1.1. Perform overall visual inspection for the serviceability of the entire raised access flooring system located in computer server rooms (Rooms 115, 116, 170 and 171).

1.1.2. Removal of damaged flooring system and accessories where it is required.

1.1.3. Tiles, pedestals, and stringers that are damaged (cracked, rusted, or corridor, etc..) beyond repair need to be replaced and installed accordance with the products listed in this SOW and Manufacturer’s specifications. For those tiles that are in a serviceable condition with only the top of the tile become loose or disbanded (screwed down), will be re-laminated in accordance with

Manufacturer’s specification. Installation of new flooring panels, stringers, pedestals, pedestal caps, and accessories shall be in accordance with the products listed in this SOW and Manufacturer’s specifications. Room 115 has approximately 20 rusted/disbonded pedestals that require replacement/rebonded. Room 116 has approximately 100 rusted/disbonded pedestals that require replacement/rebonded and 25 rusted tiles that need replacement. Room 171 has approximately 25 rusted/disbonded pedestals that need require replacement/rebonded.

1.1.4. Re-laminate the floor panels where the laminated surfacing has separated from the floor panel.

Room 170 has approximately 7 tiles that require re-laminate. Room 171 has approximately 100 tiles that need re-laminate.

1.2. The Contractor shall furnish to include but not limited to all: flooring materials, sealer, tools, equipment, supplies, accessories, etc. required removing, prepping, and installing all floor systems.

1.3. Material Disposal. Contractor shall be responsible for providing dumpster and/or hauling service.

The Government shall not furnish the Contractor with a dumpster. If a dumpster is used the Contractor shall inform the Government of the size of the dumpster needed for the job and will place the dumpster in the location as directed by the Government.

1.4. Work Locations. Whiteman Air Force Base is in Johnson County, near the town of Knob Noster, Missouri. The work occurs in building 153, Rooms 115, 116, 170 and 171. All work shall be done with the building occupied and immediate work area unoccupied. The Contractor shall coordinate all work with Contracting Officer (CO) and the occupants. Any oversize material or equipment can be brought in through the loading dock area. Storage area for equipment/materials along with a work area can be set up in the south parking lot across the street from the loading dock. Area where the work will be completed is a secure area, where no photos or recording devices are allowed.

2. Material Specifications.

2.1. Panel Construction. Except for edge panels, panel size shall be 24 by 24 inches. Finished panels shall be within a 0.010-inch tolerance of the nominal size, and shall be square within a tolerance of 0.015 inch measured corner-to-corner. The top surface of panels shall be flat within a 0.020 inch tolerance measured corner-to-corner. Panels shall be permanently marked to indicate load rating and model number. One full size panel, with the specified floor covering applied, shall be submitted after award of contract. Aluminum panels shall be of diecast construction. Steel panels shall be of die-formed construction, consisting of a flat steel top sheet welded to one or more formed steel stiffener sheets.

Panels shall be chemically cleaned, bonded, and given a baked enamel finish standard with the floor system manufacturer.

2.2. Floor Covering. There are 2 different types of tiles currently installed. Rooms 115 and 116 have the same color/style that are white with square corners with black plastic border pieces around each tile.

Room 170 and 171 have the same color/style white with a pattern. If floor coverings must be replaced, it is required to be replaced with a like color. Floor panels shall be surfaced with materials firmly bonded in place with waterproof adhesive. The floor covering shall have a volume resistivity no greater than 2 x 10 to the tenth power ohms per cubic centimeter and no less than 5 x 10 to the fifth power ohms per cubic centimeter. The electrical resistance shall remain stable over the life expectancy of the floor covering. Any anti-static agent used in the manufacturing process shall be an integral part of the material, and shall not be surface applied. Bolt heads or similar attachments shall not pierce the traffic surface.

2.3. Edge Strip. Panels shall be edged with extruded vinyl edge strips secured in place with mechanical interlock or adhesive bond, or shall be of a replaceable type. Top of strip shall be approximately 1/8 inch wide, and shall be flush with the floor surfacing.

2.4. Accessories. Registers, grilles, perforated panels, and plenum dividers shall be provided where indicated, and shall be the manufacturer’s standard type. Registers, grilles, and perforated panels shall be designed to support the same static loads as floor panels without structural failures, and shall be capable of delivering the air volumes indicted. Registers and perforated panels shall be 25 percent open area and shall be equipped with adjustable dampers.

2.5. Pedestals shall be of steel, aluminum, or a combination thereof. Ferrous materials shall have a factory-applied corrosion resistant finish. Pedestal base plates shall provide a minimum of 16 square inches of bearing surface and shall be a minimum of 1/8 inch thick. Pedestal shafts shall be threaded to permit height adjustment within a range of approximately 2 inches, to permit overall floor adjustment within plus or minus 0.10 inch of the required elevation, and to permit leveling of the finished floor surface within 0.062 inch in 10 feet in all directions. Locking devices shall be provided to positively lock the final pedestal vertical adjustments in place. Pedestal caps shall be interlock with panel stringers to preclude tilting or rocking of the panels.

2.6. Stringers shall be of rolled steel or extruded aluminum, and shall interlock with the pedestal heads to prevent lateral movement. Stringers shall be removable without the use of special tools.

3. Installation (General Information). The floor system shall be installed in accordance with the requirements specified herein and with the manufacturer’s installation instructions. Open ends of the floor, where the floor system does not abut wall or other construction, shall have positive anchorage and rigid support.

3.1. Preparation for Installation. Any computer equipment resting on the floor will be moved by in house CAE contracting personal. Any computer cable attached to the pedestal will be moved by CAE personal. The area in which the floor system is to be installed shall be cleared of all debris. Structural floor surfaces shall be thoroughly cleaned and all dust shall be removed. Floor coatings required for dust or vapor control shall be installed prior to installation of pedestals only if the pedestal adhesive will not damage the coating. If the coating and adhesive are not compatible, the coating shall be applied after the pedestals have been installed and the adhesive has cured.

3.2. Pedestals shall be accurately spaced, and shall be set plumb and in true alinement to align with floor panels. Base plates shall be in full and firm contact with the structural floor, and shall be secured to the structural floor with adhesive. Base plates shall be secured to the structural floor with steel expansion anchors after the adhesive has cured. New pedestal bases with locking collar mechanisms will replace failed and deteriorating oxidized existing material. New seismic grade, pedestal adhesive will be provided and applied throughout 100% re-bond of the pedestals throughout the space with new seismic grade adhesive.

3.3. Stringers shall be interlocked with the pedestal caps to preclude lateral movement, and shall be spaced uniformly in parallel lines at the indicted elevation. Stringers may be omitted at perimeter, columns, stairs, and ramps where panels are bolted to the pedestal caps.

3.4. Auxiliary Framing or pedestals shall be provided around columns and other permanent construction, at sides of ramps at open ends of the floor, and beneath panels that are substantially cut to accommodate utility systems. Any utility systems or building real property(pipes, conduit, etc.. will be addressed with Base CE as issues are identified.

3.5. Floor Panels shall lie flat without warp or twist, and shall bear uniformly on supports without rocking and without edges projecting above the floor plane. The panels shall be interlocked with supports in a manner that will preclude lateral movement. Only perimeter panels, cutout panels, and panels adjoining columns, stairs, and ramps shall be fastened to the floor framing. Floors shall be level within 0.062 inch measured with a 10-foot straightedge in all directions. All failed and deteriorating oxidized panels will be replaced with panels to match existing.

3.6. Cut edges of steel and wood core panels shall be painted as recommended by the manufacturer. Cut edges of composite panels shall be coated with a silicone rubber sealant or with an adhesive recommended by the panel manufacturer. Extruded vinyl edging shall be secured in place at all cut edges to prevent abrasion of cables. Where the space below the floor is a plenum, cutouts for conduit and similar penetrations shall be closed using self-extinguishing sponge rubber.

4. Principal Information

4.1. Description. The work will consist of demolishing existing damaged flooring material, removal and disposal of old material, and installing new flooring materials and accessories as specified in this

SOW.

4.2. Occupied Areas: The work to be performed on this project will include work in an occupied building. The Contractor will be responsible for damage to equipment or facilities because of negligence on the part of the Contractor. The Contractor shall coordinate the work of all activities whereby both the Government and the Contactor can continue operations with the least possible interference and inconvenience. The Contractor shall conduct all work to coordinate the means of ingress and egress from any facility that is maintained. The Contractor shall be responsible for furnishing, installing and maintaining Government approved warning tape, barricades, roped barriers, fencing, etc. to warn occupants of hazardous areas at the jobsite for the duration of the contract.

4.3. Moving Furniture and other Items. Effected equipment will be removed prior to contractor arriving to install flooring to allow for access for repair/replacement of the flooring. The Contractor will schedule installation 14 days in advance of arrival.

4.4. Base Closures. Work scheduled but not accomplished because of base closure due to weather, exercise, or actual alert will be accomplished as soon as possible after reopening of the base.

Occasionally, it is necessary to require early release of Whiteman AFB personnel, their late reporting, or even total base closure when weather conditions make normal travel unsafe. When weather conditions cause one of the above situations, Contractor personnel will be denied access to the base, unless they have been notified that their contract service has been determined essential.

Occasionally real world situations or exercises may prohibit base access or ability to remain on base.

Denied access in these situations will result in no additional costs to the Government. Information pertaining to Whiteman AFB operating status can be accessed at the following website:

4.5. Hours of Operation: Normal hours of operation shall be 0730-1530 hours, M-F, and closed on

Federally-recognized holidays, down days, and Family Days

Normally work will not be done outside these times unless special permission has been granted in advance. If holidays fall on a Saturday, the preceding Friday will be observed. If holidays fall on a Sunday, the following Monday will be observed. If a holiday falls on a schedule service day, the Contractor will be responsible for rescheduling the service for the first day post the holiday observance.

5. Additional Installation Requirements:

5.1. Warranty: Contractor is responsible for providing a one (1) year warranty on all workmanship for labor. The contractor shall manage and utilize the manufacturer’s full warranty for all material and for each product installed (i.e. carpet, tile, wall base, transitions, accessories, etc.).

5.2. Cleaning and Protection

5.2.1. The Contractor shall be responsible for removing all waste materials and debris from the installation site and discarding at a State-permitted disposal site. The Contractor shall be responsible for removing and storing all materials and equipment off base at their own expense.

5.2.2. Prior to acceptance by the Government, the Contractor shall thoroughly clean the installation area including flooring, base, walls, stairs, and landings, and surrounding area.

5.3. Building and fixture protection: The Contractor shall take all necessary action and precautions to protect the building and fixtures from damage during execution of all associated work. Fire/smoke detection equipment may need to be covered if excessive dust is produced to prevent alarming. The contractor shall coordinate all covering with the government. Covering of detection equipment shall be at no additional cost to the government. If the building or any portion thereof is damaged due to the Contractor’s operation or negligence, the Contractor shall replace or repair such, to its original condition prior to the commencement of the project. This will be done at no additional cost to the Government. Repair or replacement will be determined by the CO.

6. ENVIRONMENTAL REQUIREMENTS

6.1. The Contractor shall submit a Hazardous Handling Material Plan for review prior to the start of the project. All work shall be accomplished in accordance with Environmental Protection Agency

(EPA), and other applicable federal, state, and local regulations governing carpet, vinyl, tile flooring, ceramic tile, hazardous waste/special waste, and recycling, including Air Force Instruction (AFI) 32- 7001 (Hazardous Material Management) paragraphs 29-30 standards and in accordance with this

SOW.

6.1.1. For the purposes of this document Hazardous Materials (HM) is defined as any product, material, chemical, or substance listed in 49 Code of Federal Regulations CFR 172.101 (revised) and 40 CFR 302-304 (revised). Specifically, a HM is any substance or material, in any quantity or form that has the potential to harm human health or the environment or displays specific characteristics (reactive, corrosive, ignitable, and/or toxic).

6.1.2. Absolutely no HM shall be brought onto Whiteman AFB until that material is registered with the Hazardous Material Pharmacy (HAZMART) per Whiteman AFB Field Instruction 32-7001.

This requirement shall apply for all HM that the Contractor intends to bring onto Government property for any/all processes or applications. The Contractor shall submit a complete hazardous material inventory list including Material Safety Data Sheets (MSDS), Safety Data Sheets, (SDS) and any other supporting documentation for each and all HMs used prior to contract start or introduction of that material to Whiteman AFB. The HM inventory shall include the contract number, performance period, and a contractor point of contract for HM matters. All excess material and empty containers are the responsibility of the Contractor and shall be removed at project completion or when the container is empty. Should the Contractor’s HM requirements change during the performance period, the Contractor shall immediately notify the HAZMART of such changes in writing.

6.1.3. Storage/Use of Hazardous Materials: Only temporary storage is available at each job site. All HM to be used or temporary stored at Whiteman AFB must have the approval of and be coordinated with the 509th CED/CEV, the base Fire Department (CES/CEF), and Wing Safety. The Contractor shall observe HM management practices in accordance with AFI 32- 7286.

6.1.4. All Contractor personnel shall immediately report to the CO and the and end user any hazardous materials, chemicals, or contaminated areas encountered. Further, the Contractor personnel shall immediately cease work in area unless the work is of an emergency nature and the risk of exposure can be mitigated by the use of personnel protective equipment (PPE) or clothing. The Government will determine the best means of sampling and corrective action and will notify the Contractor accordingly.

6.1.5. The Contractor shall not use, store, or handle Class 1 ozone-depleting substances (ODS) during the course of this contract.

6.1.6. All hazardous waste or special waste generated by the Contractor during or as a result of this project shall be identified, characterized, and transported to a permitted disposal facility off base in strict accordance with the requirements of 40 CFR 260-279 (Hazardous Waste and Used Oil Regulations, Missouri Revised Statutes (Mo. Rev. Stat.) 307.177, Mo. Rev. Stat. 226.008, and 11 Missouri Code of State Regulations 30-6.010 Industrial and Hazardous Waste regulations, and Whiteman AFB Hazardous Waste management Plan. Any anticipated hazardous or special waste generation shall be identified to the CO, end user, and Base Environmental Coordinator (BEC) prior to ANY generation of such waste. The BEC will assist the Contractor with information on any special handling or storage procedures. The Contractor shall be responsible for transportation and disposal to an off-base facility at no cost to the Government.

6.1.7. The Contractor shall provide a completed Uniform Hazardous Waste manifest to document all authorized parties and locations involved in the generation, transportation, storage, and disposal of all hazardous and special wastes. The Contractor shall provide the CO, end user, and BEC a draft copy of the manifest, for review, no less than three (3) working days prior to any hazardous or special waste shipments from the installation. The BEC will sign as the generator for Whiteman AFB. The Contractor shall submit the completed original manifest to the CO, end user, and BEC not later than 30 days after waste disposal has been shipped from the base.

6.1.8. Unidentified hazardous or regulated material/waste encountered by the Contractor: The Contractor shall notify the CO and end user upon encountering any material, not identified in the contract documents, thought to be hazardous to workers, personnel in the area, or the environment. The Contractor shall immediately contact the BEC upon discovery of identified materials/waste who shall assist the Contractor in determining the appropriate procedures to be used. The Contractor shall be responsible for transportation and disposal to an off-base facility at no cost to the Government.

6.2. Asbestos:

6.2.1. Asbestos Testing of facilities will be conducted by CES personal if identified.

6.2.2. Asbestos Containing Building Materials: No asbestos containing materials shall be used on this contract. The Contractor shall certify in writing to the CO, end user, and BEC that no asbestos containing materials will be used in this contract.

6.2.3. Should the Contractor encounter previously unidentified or suspected Asbestos Containing Material (ACM) which must be disturbed to comply with the contract documents, the Contractor shall cease all work that would disturb the suspect material and shall immediately notify the CO and the end user. The Government shall take steps as appropriate, to ascertain the material’s composition and determine any remedial action.

7. GOVERNMENT-FURNISHED UTILITIES AND SERVICES

7.1. The Government will furnish utilities when available for Contactor to perform deliveries as described in this statement of work. The Contractor shall conserve use of utilities at all times.

Electricity, sewage, and water will be made available where existing. The Contractor shall ensure all employees practice utilities conservation. Any alterations or temporary connections to Government utilities shall be restored to original condition prior to the final payment for the project for which it was used. No temporary connections to available services will be allowed until the Contractor has submitted a detailed plan for the connection and has received approval from the Government for the connection prior to start of the project. The plan shall include a detailed description on how the connection will be restored to its original condition or better. Should the contractor require higher voltage or amperage than available at the building, or circuit breakers are continually tripped due to load draw the contactor may be required to provide their own power source at no additional cost to the government.

7.2. Emergency Medical Service: In the event of an emergency, dial 911. Ambulance transport responds from Whiteman AFB Clinic, Monday through Friday. On Weekends and holidays, Warrensburg ambulance transport will respond. One of the units will respond to the 911 call and if necessary, transport a Service Provider employee to one of the local hospitals. The Service Provider employee shall reimburse the respective agency for these services.

7.3. Security: The Government will provide general Security Forces service. Security Forces phone extensions are from a cellular phone 660-687-5407 for emergencies or 911 from a base phone, and 660-687- 3700 for routine calls.

8. GENERAL INFORMATION AND REQUIREMENTS

8.1. Security Requirements:

8.1.1. The Contractor, all employees, and all subcontractors shall comply with all Whiteman AFB security requirements imposed by all the local Installation Commander at all times while on the premises. All Contractor employees who will perform work on Whiteman AFB will require a criminal background check from the National Criminal Information Center performed by Whiteman AFB’s Security Forces. Failure to pass background check will prohibit entry onto Whiteman AFB. All Contractor employees must be U.S. Citizens.

8.1.2. 14 days prior to the contract start date the Contractor shall provide an up-to-date roster of all Contractor personnel to be placed on base and restricted area Entry Authorization Lists (EAL).

Employee information must include social security number, driver’s license number or state issued Identification card number, state of issue, full name exactly as shown on the ID card, date of birth, and country of citizenship. Processing time for EALs may take two (2) weeks or longer.

The Contractor must plan accordingly. The ID card information provided must be carried with the individual at all times because it will be the only form of identification that can be used for entry into restricted areas. Any changes to ID cards or vehicles must be provided to the end user as soon as possible to reprocess the paperwork for entry into restricted areas. Changes to EALs require the same processing time as the original EAL. Failure to have correct and current information on EALs will prohibit entry into restricted EALs. It is the Contractor’s responsibility to provide the end user with updated information. Should an employee no longer be employed by the company the Contractor must immediately return all base access cards to the end user or Security Forces personnel.

8.1.3. Site Security. The work area shall be restricted only to authorized personnel. These may include the Contractor’s employees, employees of subcontractors, Government employees and representatives, state and local inspectors.

8.1.4. The Contractor shall report entry into work area by unauthorized individuals immediately to the

CO.

8.1.5. The Contractor is responsible for securing all of his/her materials, tools and equipment.

8.1.6. Certain areas where work will be accomplished will require escorts, which the government will provide.

8.2. Storage Areas: Except for temporary, limited storage, at each job site, the Government will not furnish any storage areas for the Contractor’s use. In addition, the Government will not provide a site on base for any Contractor-owned office or storage facilities. It is the Contractor’s responsibility to store all materials, carpet, tile, supplies, tools, equipment, etc. at off base at no additional expense to the Government.

8.3. Safety:

8.3.1. The Contractor shall comply with all applicable federal, state, and local safety requirements. The Contractor is also required to comply with the American General Contractor’s Safety Manual and the Occupational Safety and Health Act.

8.3.2. Reporting Requirements: Contractor personnel shall report to an appropriate authority, any information or circumstance of which they are aware that may pose a threat to the security of Department of Defense (DOD), Contractor personnel, resources, and classified or unclassified defense information. The Contractor shall brief all employees upon initial on-base assignment.

8.3.3. Physical Security: Areas controlled by Contractor employees shall comply with base operations plans/instructions for FPCON procedures, random anti-terrorism measures, and local search/identification requirements. The Contractor shall safeguard all Government property including controlled forms provided for Contractor use.

8.3.4. Traffic laws: The Contractor and its employees shall comply with base traffic regulations in accordance to the provisions of AFI 31-204. Currently ONLY hands-free cell devices may be used while operating a motor vehicle.

8.3.5. Smoking Areas: While on Whiteman AFB smoking is only permitted in designated smoking areas. Contact the building facility manager for smoking areas where smoking is permitted.

8.4. Quality Control. The Contractor shall develop and maintain a quality program to ensure installation services are performed in accordance with commonly accepted commercial practices and meets the requirements of this SOW. Contractor shall develop a Quality Control (QC) Plan and implement procedures to identify, prevent, and eliminate reoccurrence of defective services. The plan shall specifically address the Contractor’s strategy to provide quality workmanship, continual process improvement, and for correcting deficiencies as required.

Specifications: All colors, patterns, and types shall be chosen by the Government from manufacturer’s standards.

Publications: The publications listed below form part of this specification to the extent referenced. The publications are referred to in the text by the basic designation only.

All carpet must comply with applicable health and safety codes, Americans with Disabilities Act Accessibility Guidelines (ADAAG) and Uniform Federal Accessibility Standards (UFAS).

In addition, the contractor is required by this contract to ensure the following Material Specifications:
