Project Title: Replace/Add Diaper Changing Stations Project Site: Kodiak CDC, Bldg. 6, JBER, Alaska

STATEMENT OF WORK

Revised

1. Background:

The Kodiak Child Development Center’s (CDC) sole mission is to provide daycare for infants to toddlers.

Kodiak has transformed from a part day program to a full-time childcare center. The existing stations need to be replaced with the stations with the ability to slide the stairs in and out from under the changing table. Also, adding an additional station in a classroom to have the ability to provide childcare to infants and toddlers.

2. Objective:

a. Contractor is to furnish all labor, materials, and equipment to accomplish the following work.

b. Contractor shall remove existing changing stations in rooms 9 and 15.

c. Contractor will remove plumbing work up to the wall during the demolition of the diaper changing stations.

d. Contractor shall install a total of 5 approved diaper changing stations with foot pedal operated sink fixtures in rooms 3, 5, 9 and 15(two in room 15).

e. Contractor will re-plumb the new sink and ensure it is operational with hot and cold water.

f. Contractor will provide standard warranty for work performed.

g. All work will be accomplished in accordance with all applicable building and fire codes in effect at the time of award of contract.

h. During the job, the contractor is responsible for compliance with all OSHA/AFOSH Standards.

i. Period of Performance is not to exceed 90 number of days from start of project.

3. Constraints:

a. The contractor’s job site is to always remain clean. Debris will be collected and disposed of daily. All old carpet, tile, and other items removed will be disposed of properly by the contractor.

b. During the performance of this project the contractor is responsible for repairing any damage incurred to existing Real Property/Grounds due to poor workmanship at no cost to the government, repair any damage caused by construction operations, and take all precautions necessary to safely protect the building and its occupants during the construction period.

c. Use of premises is limited to the indicated work area. Do not disturb adjacent areas beyond the work site.

Only the Government right to work may limit the Contractor’s use of the premises.

d. The Government will occupy facility during the entire construction period. Coordinate with the

Government during construction operations to minimize conflicts and facilitate Government usage.

e. Contractor is allowed to perform work Monday thru Friday nights from 1730-0500 or Weekends. Any work during PACAF approved family days and federal holidays will be coordinated with the Kodiak CDC director or designated point of contact (POC). Only one room will be worked on at a time, the room will have to be completed from start to finish. Extensive efforts will be made to ensure there are no airborne and debris hazards during construction that could impact the health and safety of the children in the center. Temporary protective flooring will be placed where foot traffic is most prevalent to minimize tracking hazards around the facility.

f. Contractor will need to notify the POC if the work will not be completed over the stated period.

g. Contractors will sign out a key for the building and will ensure the building is secure when not in the facility.

h. Keep driveways and entrances always serving the premises clear and available to the Government’s employee’s customers and emergency vehicles. Do not use these areas for parking or storage of materials. Additional storage requirement will be approved by the Kodiak CDC director.

i. Contractors will move furniture and equipment out of the room while working in the designated area. The center POC with designate where items can be stored. For the items that cannot be moved the contractors will cover items.

SITE VISIT: It is required that the Contractor visit the work site to verify location and thoroughly familiarize their self with existing conditions prior to submission of bid.

All work in this contract shall be done in accordance with National Fire Protection Association (NFPA), Uniform Building Code (UBC), OSHA-Occupational Safety and Health Act, EM 385-1-1, US Army Corps of Engineers Safety and Health Requirements Manual, ANSI B 20.1 1992, 40 CFR EPA, Environmental Protection Agency, and all local safety, fire and security regulations. This job requires that all personnel have appropriate, applicable and current Certificate of Fitness cards from Alaska Department of Labor.

All codes listed here shall be the latest version and most recent or up-to-date publications in print.

CONTRACTING AUTHORITY: Only the Contracting Officer is authorized to make changes or additions to the contract.

COORDINATION OF WORK: Once the contractors’ proposals have been received and reviewed, Contracting will notify the contractor awarded the contract.

Project Title: Replace Diaper Changing Stations

Statement of Work

1. Demo diaper changing stations in rooms 9 and 15

2. Demo plumbing up to the wall.

3. Install diaper changing station with built in sliding stairs complete with sink and foot pedal operated fixtures. See examples below.

Maximum width 72” minimum width 60” Maximum depth 27” minimum depth 23” Maximum Height 36” minimum height 28” Made of moisture-proof, nonabsorbent, smooth surfaces that do not trap soil and are easily sanitized. Single piece, hygienic, molded changing well with 7" retaining ledge to meet regulations with rounded corners and edges. Sturdy. DO not add any restraining straps or other objects that pose a challenge to clean and sanitize. Sinks in diaper-changing areas should be within arm’s reach of the caregivers/teachers so hand washing can be done before any other surfaces are touched and contaminated.

Child stairs must include hand-railing that leads to diaper-changing table and be sturdy. Diaper changing table must having opening at the top of the stairs so they child can climb onto the diaper changing table as a final step. Stairs will be stored in the diaper changing table.

4. Re-plumb sink and verify operation of hot and cold water.
