CDC Chamblee Campus 4770 Buford Highway Chamblee, GA 30341 DeKalb County Buildings 103, 107, and 109
I. BACKGROUND
Several locations at CDC have floorcoverings that have lasted beyond their life expectancies.
Therefore, The Government (hereinafter also referred to as ‘CDC’) is requesting procurement and construction services for floorcovering replacement. Services include the removal of existing floorcoverings, delivery & installation of new floorcoverings, lifting of existing furniture and cubicles in several CDC Buildings and campuses in Atlanta, GA. (Project locations are as noted above).
The work includes items identified below (in the SOW) and items identified in the Contract Documents provided for this project. The work of the Contractor must include the provision of all materials and labor to perform the work and to leave the work area in a clean, operationally ready condition following completion.
Some items are noted specifically on the drawings to match existing conditions without substitutions. These items must be submitted to match existing products.
Other items are noted as Basis of Design (Refer to Section III Definitions) on the drawings where the Contractor may propose the product specified or may propose equal products for consideration by CDC. Equal products must be clearly identified and clarified by the Contractor as further defined in the Contract Documents. All specifications, technical data, warranties, and samples must be submitted with Offeror’s proposal for products that are only for Basis of Design for CDC review.
II. POINT OF CONTACT
1. The Point of Contact (POC) for this procurement is the CDC (CO) Contracting Officer assigned to the project by CDC (OAS) Office of Acquisition Services.
2. All communications prior to and during the proposal phase, including any requests for clarification concerning this project must be addressed in writing to the Contracting Officer.
There must not be any direct conversations between the Contractor and the CDC Designer, COR, or other members of CDC DSSO staff prior to Contract Award.
3. Upon award of the project OAS (CO) Contracting Officer will designate a CDC (COR)
Contracting Officer Representative along with an appointment letter designating the appropriate levels of communication between the Contractor and the COR during construction.
III. DEFINITIONS
Basis of Design – Where Contract Documents name a Basis of Design, provide the specified or indicated product or an approved comparable product. The Contractor bears responsibility for vetting all non-Basis of Design products proposed to assure to CDC that they are comparable equal products. CDC will be the sole decision maker as to whether products presented are considered equals for this project.
Contractor must clearly identify any benefits to the Government for use of selecting products other than the Basis of Design.
All specifications, technical data, warranties and samples must be submitted with Offeror’s proposal.
All alternate products must be equal in specifications to basis of design products on drawings.
IV. PROJECT CONSTRAINTS INCLUDE
Contractor must follow CDC COVID Guidelines. These guidelines are published on the CDC website available to the public. These guidelines are also subject to change.
Contractor must ensure that all persons including employees and sub-contractors used for this project are US citizens or Legal Resident. Contractor must ensure all persons used for this project have the necessary security clearances and badges for proper site access. Badging includes the following items at a minimum.
1. Badging Process includes: (Refer to Specification Section 01 54 70 for additional information).
a. New Employee In-and Out-Processing
b. Background check
c. Safety Survival Skills Test
d. 1137 Credentialing Request form
e. 834 Physical Access form
2. Names of all personnel required for badging will need to be submitted within 7 days after bid award. The following items are required for the badging process. All employees are to be United States citizens or Legal Resident and have photo ID’s.
a. Employee full name
b. Employee email address
c. Employee phone number
d. Business address
e. Supervisor full name
f. Supervisor email address
g. Supervisor phone number
Contractor must perform the work in occupied work areas to remain in operation during construction. Work will require proper phasing to allow Contractor access to areas on an as needed basis. At the completion of each portion of the work the Contractor must do proper cleaning. Contractor must ensure that the spaces can safely remain occupied.
Contractor must perform the work with limited access to loading dock areas. All use of loading docks must be scheduled and approved in advance by the Contractor thru the CDC COR who in turn will obtain Building Manager approvals. Vehicles may only remain at loading docks during the period of active loading and unloading. Vehicles left beyond these periods and/or unoccupied are subject to towing and the Contractor will be subject to reprimand or removal from the site.
Storing of materials at the building sites is limited. All required materials for the work must be stored by the Contractor at an off-site facility cost to be included in the Offeror’s proposal.
Contractor must bring materials onto the site on an as needed basis. Delivery of required materials per floor and project work location is the responsibility of the Contractor. Contractor shall ensure material can be properly acclimated to the new environments prior to installation.
All trash and debris shall be removed by the Contractor from the site daily. Contractor shall not use CDC dumpsters for construction debris.
CDC is a smoke free and drug free workplace. Refer to Project Specifications Section 01 54 60 for Health and Safety requirements.
Contractor shall protect government owned equipment and property including but not limited to printers, copiers, monitors, seating, computers, doors, all furniture (fixed or loose), walls and base during performance of the work. Contractor must repair or replace any items damaged during the performance of the work.
V. PROJECT WORK AND TASKS TO BE PERFORMED
A. Project Scope:
The Contractor shall schedule work at the Chamblee Campus first and shall complete it first prior to moving to the Roybal campus. It is preferred that the Contractor not schedule work at both campuses at the same time. Order of Performance Preference is listed below for sequencing of work.
1. Chamblee Campus - Building 103: Contractor shall refer to the drawings for extent of work to be done. Contractor’s work shall include the removal of existing floorcovering.

VI. OTHER PROJECT TASKS, CLARIFICATIONS, AND REQUIREMENTS
1. Proposal: Drawings are not to be relied upon by the Contractor for final quantities of materials required, existing conditions, or full scope of requirements based on existing conditions. Contractor is responsible for conducting prior field verifications to determine the scope of work and quantities required to perform the work. Should there be any discrepancies between the field verification and the drawings the Contractor must notify the Contracting Officer immediately (prior to submitting their proposal or a soon as discovered).
2. Proposal: All specifications, technical data, warranties, and samples must be submitted with Offeror’s proposal to be considered. Contractors are required to submit a minimum of 3 and a maximum of 5 references when submitting the final proposal.
3. Proposal & Construction: Contractor is responsible for flooring preparation as required prior to performing the Work.
The CDC does not guarantee the extent of floor preparation required. Contractor shall after visiting the site ensure their proposal is a firm fixed price that covers required floor preparation.
a) Contractor shall remove existing adhesive per the manufacturer’s recommendation and shall apply new adhesive per the manufacturer’s recommendation prior to the installation of the new floorcovering.
b) At a minimum flooring preparation work shall include scraping the old adhesive off the floor so that it is smooth; no ridges, high spots or residue which would impede the application of new adhesive or prevent adhesion. Patching the substrate to even out rough spots, dips, leveling the substrate, large cracks, depressions, applying separate sealer for moisture, porosity issues or anything that may telegraph through the new flooring would be considered prep work. Contractor shall include and clarify other required prep work needs as well based on their expertise in the industry.
(One auggested floor preparation material is Schonox or one of similar type).
4. Construction: Contractor shall document furniture and adjacent space conditions before and after installation of floorcoverings via pictures and project walk through with CDC COR. Any areas of concern shall be documented in writing.
5. Construction: Contractor must repair walls, existing base, furniture, or other existing items that are damaged during construction. Repairs are at the Contractor’s expense unless otherwise documented to have been by others.
6. Construction: During Construction the Contractor must coordinate delivery and installation dates with the CDC Contracting Officer and COR in a timely manner to allow for coordination with CDC staff in the area where work is being performed.
7. Construction: Contractor shall purchase and use only manufacturer’s recommended water based low volatile organic compounds adhesive for rubber flooring, rubber base, stair nosing, stair riser, luxury vinyl tile and carpet tiles specified. Carpet tile, rubber flooring and luxury vinyl tile shall be installed with a full glue spread application per manufacturer’s recommendation. Refer to Specification Section 09 68 13 Carpet Tile for additional requirements.
8. Construction: Transitions between new flooring and existing flooring shall occur at the centerline of closed doors unless otherwise specifically noted on the Contract Documents.
9. Construction: All existing vinyl transition strips between carpet and vinyl shall be replaced with new transition strips to match existing. Provide vinyl transitions where there is a difference between floorcovering heights.
10. Construction: Contractor to coordinate and contract with the local qualified manufacturer’s representative (CWC, David Cantwell, 678-468-7869) for the removal and installation of existing DIRTT Wall system located in Building 109. Contractor shall remove and install floorcovering before existing DIRTT Wall system can be reinstalled.
All DIRTT wall work shall be included in the Contractor’s proposal and in Contractor’s Scope of Work costs.
11. Construction: Contractor shall be qualified in the use of lift systems to prevent damage to existing furniture. Contractor to submit a minimum of 3 and a maximum of 5 past projects (minimum of 75,000 square feet) using a lift system that facilitates carpet replacement. Casegoods can be lifted and in some cases slides or skates can be used to move the items. Contractor shall repair or replace any furniture damaged due to improper use of a lift system.
12. Construction: Case goods shall be lifted in all cases and in some cases the use of slides or skates may be used to move the items. Case goods currently in the buildings are from different manufacturers such as Krug, Kimball, Herman Miller, Trendway and Wrightline.
Case goods must not to be taken apart. Use the lift system to install floorcovering.
Contractor is to verify if additional furniture not listed is in the buildings.
13. Construction: Systems furniture currently in the buildings are Haworth-Race and
Herman Miller-Canvas. Furniture systems must not to be taken apart. Use the lift system to install floorcovering. Contractor is to verify if additional furniture not listed is in the buildings.
14. Construction: Existing government owned equipment (printers, phones, copiers, monitors, seating, computers, and similar items) that are connected to electrical outlets must not be disconnected.
15. Construction: Electronic and Information Technology Accessibility
a) Section 508 of the Rehabilitation Act of 1973 (29 U.S.C. 794d), as amended by the
Workforce Investment Act of 1998 and the Architectural and Transportation Barriers Compliance Board Electronic and Information (EIT) Accessibility Standards (36 CFR part 1194), require that when Federal agencies develop, procure, maintain, or use electronic and information technology, Federal employees with disabilities have access to and use of information and data that is comparable to the access and use by Federal employees who are not individuals with disabilities, unless an undue burden would be imposed on the agency. Section 508 also requires that individuals with disabilities, who are members of the public seeking information or services from a Federal agency, have access to and use of information and data that is comparable to that provided to the public who are not individuals with disabilities, unless an undue burden would be imposed on the agency.
b) Accordingly, any offeror responding to this solicitation must comply with established
HHS EIT accessibility standards. Information about Section 508 is available at http://www.hhs.gov/web/508. The complete text of the Section 508 Final Provisions can be accessed at http://www.access-aboard.gov/sec508/standards.htm.
c) The Section 508 accessibility standards applicable to this contract are: 1194.

d) In order to facilitate the Government's determination whether proposed EIT supplies meet applicable Section 508 accessibility standards, offerors must submit an HHS Section 508 Product Assessment Template, in accordance with its completion instructions. The purpose of the template is to assist HHS acquisition and program officials in determining whether proposed EIT supplies conform to applicable Section 508 accessibility standards. The template allows offerors or developers to self-evaluate their supplies and documentation detail - whether they conform to a specific Section 508 accessibility standard, and any underway remediation efforts addressing conformance issues. Instructions for preparing the HHS Section 508 Evaluation Template are available under Section 508 policy on the HHS Web site http://hhs.gov/web/508.
e) In order to facilitate the Government's determination whether proposed EIT services meet applicable Section 508 accessibility standards, offerors must provide enough information to assist the Government in determining that the EIT services conform to Section 508 accessibility standards, including any underway remediation efforts addressing conformance issues.
f) Respondents to this solicitation must identify any exception to Section 508 requirements. If a offeror claims its supplies or services meet applicable Section 508 accessibility standards, and it is later determined by the Government, i.e., after award of a contract or order, that supplies or services delivered do not conform to the accessibility standards, remediation of the supplies or services to the level of conformance specified in the contract will be the responsibility of the Contractor at its expense.
g) Electronic content must be accessible to HHS acceptance criteria. Checklist for various formats are available at http://508.hhs.gov/, or from the Section 508 Coordinator listed at https://www.hhs.gov/web/section-508/additional-resources/section-508-contacts/index.html. Materials that are final items for delivery should be accompanied by the appropriate checklist, except upon approval of the Contracting Officer or Representative.
VII. GOVERNMENT FURNISHED MATERIALS
1. Drawings: Upon written request from the Contractor and upon receipt of a signed Non-
Disclosure Agreement from the Contractor CDC will provide drawings electronically in AutoCAD 2015 or 2019 format for general reference. CDC does not validate accuracy of any documents provided. Contractor must hold CDC harmless for any documents provided. Contractor shall field verify all conditions. (A copy of the Non-Disclosure Agreement can be obtained from the CDC Contracting Officer (CO) assigned to this project.)
VIII. WORK SCHEDULES AND PERIOD OF PERFORMANCE
