Contractor shall provide comprehensive logistics and facilities support specific to participant lodging, onsite, full technology classrooms, experienced physician and subject matter expert instructors, realistic scenarios, cadaveric anatomy and procedures familiarization, professional hyper-realistic moulage with human role player(s), coordination of high angle and rescue rigging ropes instruction with practical application, modes of unconventional transportation as well as multiple dedicated training sites and medical training lanes for use. Therefore, external support will be required to sustain mission readiness through augmented contracted external facility and training support.

2. OBJECTIVE AND SCOPE

2.1. SOIDCs (10 Personnel) and CSOs (10 Personnel) from each Marine Special

Operations Company (MSOC) will conduct Professional Development Training

(PDT) and Special Operations Medical Proficiency Training, to enhance essential capabilities and proficiency of the force. The medical capability required is associated with the conduct of direct support to operations, enhance organic Role 1

Medical knowledge, skills, and abilities, and increase unit lethality through survivability in support of COMMARFORSOC and Geographic Combatant

Commanders. Total trained will be 20 personnel per course.

2.2. This training shall provide the necessary and all-encompassing medical training support to ensure that MRR possesses operational skill sets and medical capabilities necessary to accomplish its medical and tactical specific functions.

DETAILED PROGRAM REQUIREMENTS

12. BACKGROUND

In 2005, SECDEF Robert Gates mandated DOD forces in the Iraq and Afghanistan war to abide by the Golden Hour rule for MEDEVAC. Fast forward 15 years later, and these wars wage on with less MEDEVAC support and additional resource constraints.

More specifically, this stage of the GWOT is defined by shrinking medical resources, a smaller battle space footprint, and the need to perform prolonged field care for missions in evacuation delayed denied areas. Although this pivot from golden hour care and quick evacuation to prolonged field care will require significantly different equipment and medical expertise, what it most requires is a different mindset. The

GWOT has raised a generation of golden hour warrior medics, both SOCM and 18D, Marines and CSOs alike, who have been doing excellent work. They now need new skills, new equipment, and most importantly a new approach for casualty care on a new and completely different battlefield. As SECDEF James Mattis once said, "America's military has no preordained right to victory on the battlefield" (NDS, 2019). Pursuit of increased modality options for further educational advancements and training is in line with all MARSOC, SOCOM, and USMC campaign guidance in order to increase survivability efforts in future fights.

13. OBJECTIVES

13.1. This training includes objectives for training a standard baseline level of care and performance above the TCCC level for a MARFORSOC element, to include unit medical providers (MD/PA), SOIDCs, and non-medical CSOs. Training will encompass enhanced team and individual skills in the treatment and evacuation of critically injured patients. The training shall support proficiency in the 10 Core

PFC capabilities for Medics and Non-Medics as outlined by the prolonged field care working group (i.e., prolongedfieldcare.org), align with the recommendations outlined in the Joint Trauma Systems (JTS) prolonged field care clinical practice guidelines, and documentation through JTS forms and AAR reports website. The increased level of care is complex, with rapidly changing medical conditions, lifesaving interventions, and early identification of worsening symptoms. This program is focused on providing increased advanced medical knowledge, treatment options, and incorporating the basic non-medic team member to assist in the prolonged treatment of a casualty.

13.2. Increase the capability and expand the scope of surgical and critical care medical practice in order to mitigate urgent life, limb or eyesight emergencies. This includes intimate, firsthand exposure and training to standards of emergency life-saving critical care procedures, nursing, and austere surgical interventions.

13.3. Provide complete immersion in an academic medical environment.

Statement of Work (SOW)

13.4. Support an expanded scope of practice to fill identified gaps per 2015 USASOC

Prolonged Field Care exercise After Action Report and the 2016 Prolonged Care

Capabilities Based Assessment to include protocol development, improved pharmacology training, improved patient care skills, improved identification and management of critical care illnesses, increased critical care teleconsultation proficiency, improved familiarity with diagnostic, treatment, and monitoring technologies (ultrasound, ventilators, pulse oximetry, end-tidal CO2 monitors), and increasing use of high-fidelity simulation resources.
