SCOPE OF WORK
Remove and Replace Concrete at Bldg. 7 Loading Dock Waco VA Project
1. Requirements
The contractor shall be responsible for providing all labor and equipment required for the services to remove and replace concrete at Building #7 Loading Dock Area. All work performed will be done at the Doris Miller VAMC in Waco, at 4800 Memorial Drive, Waco, Texas 76711. After all work is completed, the contractor will have a walkthrough with a VA representative to ensure all work is completed and ready for invoicing. The contractor’s period of performance (POP) is 121 days from the notice to proceed (NTP). This period of performance includes all lead times.
The contractor shall provide all personnel, services, vehicles, transportation, tools, equipment, facility supervision, administration supervision and any other item(s) necessary to perform and accomplish this service. The contractor shall be familiar with all the terms, conditions, and requirement herein contained.
Note: “The C&A requirements do not apply, and that a Security Accreditation Package is not required for this contract.”
2. Project Requirements
a. Demo of Existing Concrete
1. The contractor shall provide all barriers, signage and safety equipment necessary to prevent VA employe and patient access during construction.
2. Dust abatement shall be provided during the demo process, and it shall be the sole responsibility of the contractor to insure all employees use proper Personal Protective Equipment (PPE) while performing this work.
3. Existing metal guard rails are to be removed and stored for refurbishment and reinstallation.
4. Approximately 695 SF of existing reinforced concrete slab shall be broken up and removed.
5. Demoed concrete and metal reinforcement shall be taken off site and disposed of in accordance with local, state and federal requirements.
b. Construction of New Concrete Slab
1. Contractor shall loosen existing base material by raking, remove organics, adjust grading, and install new washed 3/4minu base materials. Base material should then be mechanical compacted to 90% proctor.
2. Install form work with releasing agent, ensure forms are set so water flows to catch basin.
3. Inspect existing catch basin and drain lines for damage or defect. If damaged is identified notify COR to determine repair or replacement requirements. Adjust catch basin height as required to insure proper drainage and installation.
4. Install 1/2inc dia. rebar on chairs at midpoint of concrete, 12inc on center each way.
5. Place 3500PSI concrete 5” deep with 8inc turndown edges. Provide control joints
20ft on center each and slope to drain. Where new concrete pour abuts adjacent slab/concrete structure drill and insert 12inc long, 1/2inc dia. rebar dowels at 12inc on center to adjacent concrete. Provide 1/2inc notched fiber expansion board between new and existing concrete.
6. Finish new concrete slab with light broom finish and tooled edges.
7. Should there be excess concrete do not dispose of this material on site. All spoilage must be disposed of offsite in accordance with local, state and federal regulations.
8. Existing stored metal handrails should be stripped of all paint and primed to receive new paint. New finish coat should match existing safety yellow paint that was removed.
9. Install refinished existing handrails.
10. Remove all signage, barriers and excess materials. The site shall be cleaned and damaged landscaping restored to preconstruction conditions.
3. Government Holidays
a. The Government hereby provides notice and Contractor hereby acknowledges receipt that Government personnel observe the listed days as holidays:

b. In addition to the days designated as holidays, the Government observes the following days:
a. Any other day designated by Federal Statute
b. Any other day designated by Executive Order
c. Any other day designated by the President’s Proclamation
It is understood and agreed between the Government and the Contractor that observance of such days by Government personnel shall not otherwise be a reason for an additional period of performance, or entitlement of compensation. In the event the Contractor's personnel work during the holiday, they may be reimbursed by the Contractor, however, no form of holiday or other premium compensation will be reimbursed either as a direct or indirect cost, other than their normal compensation for the time worked.
4. Invoices Submit the proper documentation and reports to receive payment on this service. See information below:
• Payment to be made monthly, in arrears, following satisfactory performance by Contractor’s personnel and upon receipt of a properly prepared invoice.
• Contractor shall be required to invoice through the Tungsten System at http://www.tungsten-network.com/us/en/veterans-affairs/. Additional information regarding Tungsten shall be provided upon award.

• The contract shall have current registration in the System for Award Management (SAM) for this solicitation; URL: www.sam.gov.
• The invoice MUST be itemized to include the following information. Any information listed below, and not provided on an invoice, will render that invoice incomplete and the invoice will be returned for immediate correction.
• Time period being invoiced for.
• Description of services performed.
• Unit Cost billed.
• Extended amount due.
• Invoice number, date.
• Contract and obligation numbers.
5. Other Considerations
• The contractor shall be responsible for the verification of all requirements for this service.
All measurements and conditions must be verified by the contractor. Any discrepancies between field conditions, project documents, and this SOW shall be immediately brought to the Contracting Officer’s attention.
• The contractor’s POP is 121 days from the NTP.
• Majority of the work shall be performed during normal working hours unless other wised noted by the COR. Normal working hours are 8:00 am – 4:30 pm, Monday – Friday.
• The contractor’s supervisor shall always be available to address any needs the VA COR may have during the POP which is 1 year.
• Contractor shall provide signage, fencing/barriers, ground guides, flagging and PPE as required to complete the work as described in this Scope of Work (SOW).
6. Codes and Standards
The project shall be in accordance with the latest International Building Code (IBC), National Fire Protection Associate (NFPA) standard, Uniform Federal Accessibility Standard (UFAS), ASTM Standards, American Concrete Institute (ACI), VA Specifications/Standards, and local facility requirements. In the event there is a conflict between any of the standards/codes, the more stringent standard/code shall apply.
The contractor shall provide all applicable safety signage and barricades applicable to OSHA, VHA and DOD regulations. Contractor shall comply with VHA DIRECTIVE 2004-012 and all VA Center Policy Memorandums, including but not limited to the following:
a. Protection of patients, visitors, and employees from traumatic injury, as well as occupational and facility–associated infections.
b. Compliance with OSHA and State construction safety regulations. Construction contracts require that the general contractor’s on-site “competent person” have completed the OSHA 30-hour construction worker course and that all other general and sub-contractor’s workers have completed the OSHA 10-hour construction worker course or other relevant competency training. The determination for training is based on the project hazards and complexity, State and Federal regulations and VA requirements. Verification of completion of training will be required prior to performing work.
c. Compliance with EPA and State environmental regulations.

7. Safety Plan Requirements Prior to performing any work, the contractor will be required to submit a safety plan in accordance with OSHA and VA Regulations.
The safety plan shall include, but not limited to the following:
A. Safety and Health Policy (i.e., company policy) B. Safety and Health Objectives (i.e., company goals, procedures, etc) C. Job Site Inspections (i.e., safety inspection/potential hazards/prevention, etc) D. Contractor Safety Administrator (i.e., competent person/safety officer) E. Personal Protective Equipment (i.e., head, hearing, and eye protection, footwear, clothing, gloves, etc.)
F. Safety Discipline (i.e., correction of violations, etc) G. Hot Work Permit (i.e., responsibilities, procedures, etc) H. Emergency Procedures (i.e., responsibilities, procedures, etc) I. Clothing
• PPE is available and properly worn.
• Clothing is relatively dust free with the construction area and patient areas.
